Occupational therapy interventions for children from Ukrainian war zones using ESDM (The Early Start Denver Model)

Abstract:

Cíl a podstata práce: Cílem této bakalářské práce je seznámení odborné i laické veřejnosti s využitím metodiky The Early Start Denver Model (dále ESDM) u dětí v raném a předškolním věku, pocházejících z ukrajinských válečných zón s poruchou autistického spektra a diagnostikovanou posttraumatickou stresovou poruchou. Bakalářská práce se zabývá využitím metody ESDM, tedy Denverské metody rané intervence v ergoterapeutické praxi dětí z ukrajinských válečných zón. Práce se dělí na dvě části. Teoretická část práce, tedy přehled teoretických poznatků, je zaměřena na podrobné vysvětlení metody ESDM. Dále je zde charakterizována role ergoterapeuta v rámci multidisciplinárního týmu pracujícího podle metody ESDM, popis aplikace metody v ergoterapeutické praxi, tedy způsob, jakým ergoterapeut pracuje s dětmi z ukrajinských válečných zón s posttraumatickým stresem, u kterých byla diagnostikovaná porucha autistického spektra (dále PAS), a vztah k všedním (denním) činnostem u těchto dětí. Zároveň je v rámci této části také objasněna definice poruch autistického spektra a posttraumatického stresu u výše zmíněných dětí, které uprchly před válkou do země s odlišným mateřským jazykem. Metody: Hlavní část je tvořena dvěma kazuistikami demonstrující využití ESDM v rámci ergoterapeutické praxe. Pozorovány a hodnoceny byly dvě děti ve věku 30 měsíců a 56 měsíců. Obě mají diagnostikovanou PAS a PTSR, a také společné příznaky, které v předválečné době nebyly evidované: opakovaná autostimulace v podobě masturbace, poruchy spánku, strach z vody, odmítání jídla a epizody agrese. S vybranými klienty jsme s ESDM terapeutkou již od června pracovali v rámci charitativního programu na pomoc dětem uprchlíků. Od listopadu pak ergoterapeutická intervence probíhala individuálně 60 minut 1x týdně ve zdravotnickém zařízení v Praze. ESDM terapie se konala 2x týdně ve zdravotnickém zařízení a 2x týdně na hřišti venku. Náš multidisciplinární tým pracoval na dosažení stejných cílů, především regulace emočního stavu dítěte, zmírnění autostimulace, zlepšení spánku, nácvik ADL, rozvoj hrubé a jemné motoriky, senzorických, kognitivních funkcí formou hry a pomoc s integrací v cizí zemi. Výsledky: Porovnaní vstupního a výstupního vyšetření na základě cílů rehabilitace poukazuje, že ergoterapeutická intervence přispěla ke zlepšení očního kontaktu a soběstačnosti u zmíněných dětí. Závěr: Z výsledků hodnocení v praktické části této práce vyplývá, že díky spolupráci s ESDM terapeutem došlo ke zlepšení udržení očního kontaktu, komunikace, zmírnění autostimulace a děti již neměly problém s umýváním rukou.

Abstract:

The aim and essence of the work: The aim of this bachelor's thesis is to familiarize specialists and the general public with the use of The Early Start Denver Model (hereinafter ESDM) methodology with Ukrainian war zones children of early and preschool age with autism spectrum disorder and diagnosed post-traumatic stress d sorder. The bachelor's thesis deals with the use of the ESDM method, i.e. the Denver method of early intervention in the occupational therapy practice with children from Ukrainian war zones. The work is divided into two parts. The theoretical part of the thesis, i.e. an overview of theoretical knowledge, is focused on a detailed explanation of the ESDM method. It is followed by an overview of the role of an occupational therapist within a multidisciplinary team working according to the EDSM method, specifically in cases of working with children with post-traumatic stress, coming from Ukrainian war zones. It concerns children diagnosed with autism spectrum disorder (hereinafter ASD) and their everyday activities. At the same time, this section also clarifies the definition of autism spectrum disorders and post-traumatic stress in the abovementioned children who fled before the war to a country with a different mother tongue. Methods: The main part consists of two case studies demonstrating the use of ESDM in occupational therapy practice. Two children aged 30 months and 56 months were observed and assessed. Both have been diagnosed with ASD and PTSR, as well as common symptoms that were not recorded in the pre-war period: repeated self-stimulation in the form of masturbation, sleep disorders, fear of water, refusal of food and episodes of aggression. The ESDM therapist and I have been working with selected clients since June 2022 as part of a charity program to help refugee children. Since November 2022, the occupational therapy intervention took place individually for 60 minutes once a week in a medical facility in Prague. ESDM therapy took place twice a week in a medical facility and twice a week on the playground outside. Our multidisciplinary team worked to achieve the same goals, mainly regulation of the child's emotional state, reduction of self-stimulation, improvement of sleep, ADL training, development of gross and fine motor skills, sensory and cognitive functions in the form of play and assistance in integration in a foreign land. Results: A comparison of the entrance and exit examination based on the rehabilitation goals shows that the occupational therapy intervention contributed to the improvement of eye contact and self-sufficiency in the abovementioned children. Conclusion: The results of the evaluation in the practical part of this work show that due to the cooperation with the ESDM therapist, there was an improvement in maintaining eye contact, communication, a reduction in self-stimulation, and the children no longer had a problem with washing their hands.
