Effect of clown therapy for pediatric patients

Abstract:

Cíl: Cílem diplomové práce bylo zjistit vliv zdravotních klaunů na pediatrické pacienty během hospitalizace ve vybraných nemocnicích. Dílčími cíli bylo zjistit míru úzkosti u dětí před operací, obavy dětí před operací a vnímání bolestí po operaci. Srovnávaly se hodnoty mezi intervenční (dětí s intervencí zdravotního klauna) a kontrolní skupinou (dětí bez intervence zdravotního klauna). Práce zkoumá také vliv demografické charakteristiky na vnímání míry úzkostí před operací, obavy před operací a vnímaní bolestí po operací. Metodika: Soubor výzkumných osob tvořilo 157 pediatrických pacientů z dětských oddělení nemocnic Olomouckého, Moravskoslezského a Jihomoravského kraje. Intervenční skupinu tvořilo 82 dětí a kontrolní skupinu tvořilo 75 dětí. Výzkumné šetření bylo provedeno metodou kvantitativního výzkumu formou dotazníků a měřících nástrojů. Konkrétně byly použity dva typy dotazníků - standardizovaný dotazník m-YPAS, který měří míru úzkostí dětí před operací a dotazník CSWQ který slouží k hodnocení obav dětí před operací. Měřících nástrojů (konkrétně FLACC, VAS a Obličejová škála bolesti) bylo využito při posuzování vnímání bolesti u dětí po operaci. Výsledky: Na základě analýzy výsledků byl prokázán pozitivní vliv zdravotních klaunů na pediatrické pacienty. Bylo potvrzeno, že zdravotní klauni snižují předoperační úzkost u dětí, dětí mají menší obavy před operací a většina dětí vnímá i menší bolest po operaci. Ukázalo se, že demografické údaje (věk, pohlaví, čas výkonu, druh výkonu, další onemocnění) nemají signifikantní vliv na vnímání úzkostí, obav před operací a vnímání bolestí po operaci. Závěr: Výsledky výzkumu prokázaly signifikantní rozdíl mezi intervenční a kontrolní skupinou. Intervence zdravotního klauna snižuje míru úzkosti, obavy před operací a bolest po operaci.

Abstract:

Aim: The aim of the diploma thesis was to determine the influence of medical clowns on pediatric patients during hospitalization in selected hospitals. The partial goals were to find out the level of anxiety in children before surgery, the fears of children before surgery and the perception of pain after surgery. The values between the intervention group (children with medical clown intervention) and the control group (children without medical clown intervention) were compared. The work also examines the influence of demographic characteristics on the perception of anxiety before surgery, fear of surgery and the perception of pain after surgery. Methodology: The group of researchers consisted of 157 pediatric patients from the children's wards of the hospitals of the Olomouc, Moravian-Silesian and South Moravian regions. The intervention group consisted of 82 children and the control group consisted of 75 children. The research was carried out using the method of quantitative research in the form of questionnaires and measuring tools. Specifically, two types of questionnaires were used - the standardized m-YPAS questionnaire, which measures children's anxiety before surgery, and the CSWQ questionnaire, which is used to assess children's worries before surgery. Measuring tools (specifically FLACC, VAS and Facial Pain Scale) were used to assess pain perception in children after surgery. Results: Based on the analysis of the results, a positive effect of medical clowns on pediatric patients was demonstrated. It has been confirmed that medical clowns reduce preoperative anxiety in children, children have less anxiety before surgery, and most children perceive less pain after surgery. Demographic data (age, gender, time of operation, type of operation, other diseases) have been shown not to have a significant effect on the perception of anxiety, anxiety before surgery and the perception of pain after surgery. Conclusion: The research results showed a significant difference between the intervention and control groups. Medical clown intervention reduces anxiety, anxiety before surgery, and postoperative pain.
