Acute pain in the postoperative care of patients after total hip joint replacement

Abstract:

Cíl: Cílem diplomové práce je porovnat intenzitu subjektivně vnímané akutní bolesti u pacientů po totální náhradě kyčle v pooperačním období při hospitalizaci na ortopedickém oddělení ve Fakultní nemocnici Ostrava. Zhodnotit vztah mezi vnímáním akutní pooperační bolesti a determinanty, jako jsou věk, předoperační užívání analgetik, rehabilitační režim a edukací. Metodika: Soubor tvořilo 123 pacientů hospitalizovaných na ortopedickém oddělení ve Fakultní nemocnici Ostrava. Pro sběr dat byl použit standardizovaný dotazník Mc Gillovy university doplněný o otázky zaměřené na zvolené dílčí cíle. Dotazník měl dvě části, kdy první část byla pacientovi předána k vyplnění v den vertikalizace a druhá část v den propuštění. Pro analýzu získaných dat byla použita popisná statistika, a to frekvenční tabulky s absolutními a relativními počty, medián, aritmetický průměr, směrodatná odchylka. Testování hypotéz bylo provedeno pomocí párového Wilcoxonova testu. Pro vyhodnocení dalších hypotéz byl použit Kruskal-Wallisův test pro více výběrů a Wilcoxonův test pro dva výběry. Statistické testy byly hodnoceny na hladině významnosti 5%. Při zpracování dat byl použit program STATA verze 13. Výsledky: Na základě získaných dat byl zjištěn významný rozdíl mezi vnímáním akutní pooperační bolesti v den vertikalizace a v den propuštění pacienta, kdy akutní pooperační bolest je největší v den vertikalizace. Pacienti, kteří pravidelně předoperačně užívali analgetika, udávali větší akutní bolest při propuštění. Největší omezení způsobené akutní pooperační bolesti, pociťovali pacienti v době vertikalizace i v období propuštění v oblasti rehabilitace. Pacienti, kteří byli v rámci léčby bolesti edukováni lékařem, pociťovali nejmenší pooperační bolest. Akutní bolest v době vertikalizace byla největší u pacientů, s věkem menším než 65 let. V době propuštění, pak úměrně s věkem intenzita akutní bolesti stoupá. Závěr: Zdravotničtí pracovníci by se měly vzdělávat v oblasti léčby bolesti a používat nejnovější výsledky studií ve zmirňování a odstraňování bolesti, protože tím snížíme nejen utrpení pacienta, ale významně ovlivníme kvalitu jeho života. Abstract:

Objective: The aim of this thesis is to compare the intensity of the subjective perception of acute pain at patients passed total hip replacement in the postoperative period during hospitalization at the orthopedic department at the University Hospital Ostrava. Another point of view is to evaluate the relation between the perception of acute postoperative pain and determinants such as age, preoperative use of analgesics, rehabilitation and the education system. Methods: The sample consisted of 123 patients hospitalized in the orthopedic department at the University Hospital Ostrava. For data collection was used a standardized questionnaire Mc Gill University, accompanied by questions focused on selected targets and had two parts, the first part of the questionnaire was handed to the patient to fill in during the verticalization and the second part during the discharge. For the analysis of the data was used descriptive statistics and frequency tables with absolute and relative numbers, median, arithmetic mean, standard deviation. Hypothesis testing was performed by using paired Wilcoxon test. For the evaluation of other hypotheses was used Kruskal-Wallis test for multiple selections and Wilcoxon test for two selections. Statistical tests were evaluated on a significance level of 5%. In data processing software was used STATA version 13. Results: The findings based on the data are these, there is a significant difference between the perception of acute postoperative pain on the day of verticalization and the day of discharge of the patient. The acute postoperative pain is the biggest on the day in the verticalization. Patients who used preoperative analgesics regularly reported bigger acute pain at discharge. The biggest limitation was felt patience during the verticalization and rehabilitation caused by acute postoperative pain. Acute pain during verticalization was greatest at patients with age up to 65 years. The accute pain is bigger with the growing age in the time of discharge. Conclusion: Health professionals should be educated in pain management and use the latest results of studies in alleviating and eliminating pain, not only because it will reduce the suffering of patients, but significantly will affect the quality of life.
