Lateral epicondylitis of the elbow: comparative study of shockwave rehabilitation and shockwave rehabilitation with deep transverse massages.

Abstract:

Epicondyles tendons are special: like the supraspinatus or Achilles tendon, they are less vascularized than other tissues. Degradation of these tissues causes an inflammation due to the decrease in oxygen supply (anoxia) in the area. This inflammation causes micro traumas and a decrease of patient?s functional ability. Diagnosis of an elbow?s lateral epicondilytis is based on the following clinical examination: - Pains in case of touching the lateral Epicondyle; - Pains in case of passive extension of the elbow if combined with palmar flexion of the wrist; - Positive result of both Mill and Thomson tests; - Pains during extension of both the wrist and elbow. In this study we will compare the efficacy of two treatments; we will take a sample of 20 patients, men and women aged 40 to 50 who regularly practice a physical activity. We will divide them into two groups. Group ?A? will be treated by chock waves according to STORZ Medical protocol and Group ?B? will be treated with chock waves according to the same protocol of Group ?A? and with deep transverse massages (M.T.P.). The patients will be divided into each group randomly. Our patients will correspond to the principles of inclusion of the study. Shock waves treatments as well as deep transverse massages will be carried out once a week. Results will be analysed at the end of the fifth week of treatment. We will able to realize graphics with the real values. These graphics will be set in accordance to the pain of patient and his/her rate of functional recovery. At the end of 5 weeks of treatment, we can notice that the results obtained by the patients of group ?B? are more considerable that the results obtained by the patients of group ?A?. The rate of functional recovery of group ?B? is increased of 39% in respect to an increase 27% of group ?A?. We will also verify that the pain of the patients of group ?B? was reduced in a more considerable way in respect to the pain of the patients of group ?A?. Abstract:

I tendini dell?epicondilo hanno una particolaritŕ : come il sovraspinato o il tendine di Achille, sono meno vascolarizzati rispetto ad altri tessuti. La degradazione dei tessuti provoca una infiammazione a causa della diminuzione del flusso di ossigeno (anossia) nella zona. Tale infiammazione provoca micro-traumi ed una diminuzione dell?attivitŕ funzionale del paziente. La diagnosi dell?epicondilite laterale del gomito si basa sull?esame clinico seguente: - Dolori alla palpazione dell?epicondilo laterale. - Dolori durante l?estensione passiva forzata del gomito associata alla flessione palmare del polso. - Un risultato positivo dei test di Mill e Thomson. - Dolori durante l?estensione combinata del gomito e del polso. In questa tesi confronteremo l?efficacia dei due trattamenti. Sceglieremo 20 pazienti uomini e donne, etŕ dai 40 ai 50 anni, che praticano una attivitŕ fisica regolare. Li divideremo in due gruppi. Il gruppo ?A? sarŕ trattato con onde d?urto secondo il protocollo STORZ Medical ed il gruppo ?B? sarŕ trattato con onde d?urto secondo lo stesso protocollo del gruppo ?A? in aggiunta a massaggi transversi profondi (M.T.P.). La divisione dei gruppi sarŕ effettuata in modo aleatorio sui nostri pazienti che corrispondono ai criteri di inclusione della tesi. Effettueremo sedute di onde d?urto una volta alla settimana. I massaggi transversi profondi saranno effettuati anch?essi una volta a settimana. Analizzeremo i risultati al termine di cinque settimane di trattamento e potremo creare grafici con i valori reali. Tali grafici saranno stabiliti sul dolore del paziente ed il suo tasso di recupero funzionale. Al termine delle cinque settimane di trattamento, potremo notare che i risultati ottenuti dai pazienti del gruppo ?B? sono piů significativi di quelli ottenuti dai pazienti del gruppo ?A?. Un incremento del tasso di recupero funzionale del gruppo ?B? del 39% rispetto ad un incremento del gruppo ?A? del 27%. Constateremo anche che il dolore dei pazienti del gruppo ?B? diminuisce in modo piů significativo rispetto a quello dei pazienti del gruppo ?A?.
