Orthorexia Nervosa: Pathological fixation on eating healthy food

Abstract:

Diplomová práce se zabývá patologickým zaujetím zdravým stravováním, označovaným jako orthorexia nervosa. Práce si klade za cíl představit koncept ortorexie, určit specifika této poruchy vzhledem ke stávajícím kategoriím duševních poruch uvedených v DSM-IV a MKN-10 a ověřit jedinečnost této poruchy jako svébytné diagnostické kategorie. V teoretické části práce byla popsána historie pojmu, definice a diagnostická kritéria poruchy, její klinický obraz, vybrané psychologické a sociokulturní kontexty vzniku poruchy, epidemiologie, nástroje pro zjišťování ortorexie a možnosti terapie. Bylo provedeno porovnání ortorexie s mentální anorexií, obsedantně-kompulzivní poruchou, fobiemi a návykovými a impulzivními poruchami, resp. nedrogovými závislostmi. Ze srovnání bylo zjištěno, že ortorexie se nejlépe shoduje s pojetím nedrogových závislostí, z nichž se významně podobá workoholismu. Na základě jedinečnosti projevů ortorexie byl navržen model ortorexie jako poruchy sui generis s vlastními diagnostickými kritériemi a definicí. Navržená diagnostická kritéria byla následně ověřena praktickým výzkumem. Pro vytipování respondentů do výzkumu byl použit modifikovaný test autora termínu ortorexie Stevena Bratmana. Zvýšená pozornost byla věnována úpravám testu vzhledem k dílčímu cílu výzkumu, jímž bylo ověřit cut-off skóre Bratmanova testu. Dotazník byl administrován 132 respondentům z populace osob, jež se zajímají o zdravé stravování. Pro vlastní výzkum byl vytvořen strukturovaný rozhovor, který byl uskutečněn s 21 osobami, z nichž 10 bylo shledáno ortorektickými. Na základě výzkumu lze považovat ortorexii jako svébytnou duševní poruchu za prokázanou a za vhodný předmět dalšího zkoumání.

Abstract:

This thesis deals with a pathological fixation on eating healthy food, known as orthorexia nervosa. Thesis aims to introduce the concept of orthorexia, determine the specifics of this disorder in light of existing categories of mental disorders listed in DSM-IV and ICD-10 and verify the uniqueness of this disorder as a distinct diagnostic category. The theoretical part describes the history of the concept, definition and diagnostic criteria for the disorder, the clinical picture, selected psychological and socio-cultural contexts of the disorder, epidemiology, detection tools and treatment possibilities of orthorexia. A comparison of orthorexia was made with anorexia nervosa, obsessive-compulsive disorder, phobias and addictive and impulsive disorders, respectively non-drug addictions. The comparison revealed that orthorexia significantly coincides with the concept of non-drug addictions, and that it is substantially similar to workaholism. Based on the uniqueness of displays, there was created a model of orthorexia as peculiar disorder with its own diagnostic criteria and definitions. Proposed diagnostic criteria were subsequently reviewed in a practical research. For the selection of respondents to the research was used modified test made by the author of the term orthorexia Steven Bratman. Increased attention has been paid to modify the test in respect to a part of the research objectives, which was to prove the cut-off score of the Bratman’s test. The questionnaire was administered to 132 respondents in the population of people who are interested in healthy eating. Own research has a form of a structured interview and was conducted with 21 persons, of which 10 were found orthorexic. Based on the research was proved that orthorexia can be regarded as specific mental disorder and that it is a suitable subject for further investigation.
