A medium-term result after the acute treatment of ischemic vascular stroke

Abstract:

Práce je zaměřena na zjištění funkčních neurologických následků u pacientů, kteří prodělali cévní mozkovou příhodu. Teoretická část práce popisuje patofyziologii zejména prokrvení mozku, druhy cévních mozkových příhod, neurologické vyšetření a nejmodernější dostupnou terapii v oblasti ischemických příhod. V praktické části práce jsou analyzovány výsledky telefonických rozhovorů s neurologicky postiženými pacienty nebo jejich rodinnými příslušníky. Diplomová práce se snaží zjistit, zda onemocnění cévní mozková příhoda ovlivňuje funkční stav pacientů. Praktická část byla provedena metodou kvantitativního výzkumu. Vstupní data byla získána pomocí telefonických polostrukturovaných rozhovorů. Výzkumné setření probíhalo v období leden 2017 až prosinec 2017 prostřednictvím databáze pacientů Centra vysoce specializované cerebrovaskulární péče. Rankinova hodnoticí škála má za úkol zhodnotit funkční stav u pacientů, kteří prodělali CMP a využívá se v klinických studiích. Modifikovaná škála důsledků cévní mozkové příhody je použitelná k orientačnímu hodnocení disability, především mobility a má 6 úrovní: 0 – bez příznaků, až 6 – úmrtí. Výsledky rozhovorů slouží k dalšímu zpracování samotnému KCC (kromě jiného školení zdravotnických záchranářů). Analýza a interpretace dat. Výzkumný vzorek tvořilo 342 pacientů, kteří se k definitivní péči ošetření po cévní mozkové příhodě dostali do KCC. Úspěšně zkontaktovaní respondenti se pohybovali ve věkovém rozpětí 36 – 92 let. Z provedených intervenčních metod kralovalo provedení intravenózní trombolýzy v počtu 72 %. 89 % z dotázaného vzorku využili služeb zdravotnické záchranné služeb. Naopak jen 22,5 % pacientů využili služeb rehabilitace (v posthospitilazační fázi v rámci neurologického oddělení) a výsledek tak stojí za zvážení. Z nejzásadnějších výsledků nebyl zjištěn statisticky významný rozdíl v míře funkčního postižení modifikované Rankinovy škály (mRS) po 12 měsících od vzniku příhody a pohlaví pacientů. Z dalších neméně důležitých výsledků je potřeba definovat, že u pacientů ve věkové kategorii 60+ zůstává horší funkční reziduum po ischemické formě CMP v rámci mRS než u pacientů mladších. Pacienti, kteří využili služeb ZZS, zaujmuli ve škále 18,3 % u postižení mRS-0, nejvíce 30,2 % u postižení mRS-1. Dále se postižení progresivně snižuje. Naopak ti, kteří služeb nevyužili jsou novnoměrně rozloženi s dominancí mRS-1 a mRS-3, onemocnění zde zasahuje až do škály mRS-4 a dokonce byl zaznamenán jeden respondent a aktuálním průběhem čerstvě probíhající CMP. V závěru je kladen důraz na mezioborovou spolupráci, zejména se ZZS a následnou rehabilitaci.

Abstract:

This thesis is focused on a detection of functional neurological consequences for the patients who have hade a stroke. The theoretical part (of the thesis) describes the pathophysiology, in particular the blood supply of the brain, types of strokes, neurological examination and the most advanced therapy available for the field of cerebral vascular events. The practical part (of the thesis) concerns the results of interviews on the telephone with neurologically affected patients or their family members. The diploma thesis tries to find out whether the cerebral vascular disease affects the functional condition of the patients. The practical part was done by the method of quantitative research. Input data was obtained by telephone semi-structured interviews. The research was conducted between January 2017 and December 2017 through a patient database of the Center of Highly Specialized Cerebrovascular Care. The Rankin rating scale (Modified Rankin Scale, mRS) is designed to evaluate the functional status of patients who have undergone ictus cerebri and are used in clinical trials. A modified range of stroke consequences is applicable to the orientation assessment of disability, particularly mobility, and has 6 levels: 0 - no symptoms, up to 6 deaths. The interview results are used for further processing by the Center of Highly Specialized Cerebrovascular Care itself (including training of health rescuers). Analysis and interpretation of data. The research sample consisted of 342 patients who went to the Center of Highly Specialized Cerebrovascular Care for definitive post-stroke treatment. Successfully contacted respondents ranged from 36 to 92 years of age. 72 % of the intravenous thrombolysis was performed on the interventional methods performed. 89 % of the sample surveyed used the emergency services. On the other hand, only 22.5 % of the patients used rehabilitation services (in the post-hospitalization phase within the neurological department) and the result is worth considering. No statistically significant difference in functional impairment scores of the modified Rankin Scale (mRS) was observed from the most basic results 12 months after the onset of the event and the sex of the patients. Of the other less important results, it is necessary to define that the worse functional residue remains after the ischemic form of ictus in mRS than in younger patients in patients aged 60+. Patients who received paramedic services scored 18.3 % for mRS-0, and no more than 30.2 % for mRS-1. In addition, the disability progressively decreases. On the contrary, those who have not used the services are newly spread out with the dominance of mRS-1 and mRS-3, the disease here extends up to the mRS-4 scale and even one respondent and the current course of the newly running ictus have been recorded. At the end, emphasis is placed on interdisciplinary cooperation, especially with paramedic services and subsequent rehabilitation.
