Audit of electronically managed nursing documentation

Abstract:

Cíl: Cílem diplomové práce je pomocí auditního šetření zjistit, zda úroveň kvality elektronicky vedené ošetřovatelské dokumentace u pacientů starších 65 let odpovídá aktuálně zavedeným standardům ve vybraném zdravotnickém zařízení, a zda elektronická ošetřovatelská dokumentace jako celek reflektuje reálný stav potřeb pacientů starších 65 let i z pohledu sester. Metodika: Audit byl proveden metodou inspekce uzavřené komplexní ošetřovatelské dokumentace na pracovištích: Následná péče, Interna a kardiologie, Kardiochirurgie, Rehabilitace, Urologie. Byl vytvořen auditní list se 75 kritérii pokrývajícími většinu možností záznamů. Bylo auditováno 100 dokumentací pacientů starších 65 let. Doplňkem auditu byl kvantitativní výzkum realizovaný nestandardizovaným anonymním dotazníkem, vytvořeným na základě specificity záznamů u pacientů starších 65 let, určeným sestrám pracujícím na auditovaných pracovištích. Výzkumný soubor tvořilo 116 respondentů. Obě šetření probíhala od prosince 2021 do ledna 2022. Výsledky: Auditem bylo zjištěno, že na pracovištích je rozdíl mezi kvalitou vedení dokumentace pacientů starších 65 let a pacientů všech věkových skupin. Kvalita vedení se liší i mezi pracovišti, a klesá se vzrůstající délkou hospitalizace. Dotazníkovým šetřením bylo zjištěno, že mezi pracovišti není rozdíl v názoru sester na vhodnost přidání položek do elektronické ošetřovatelské dokumentace pacientů starších 65 let. Nabízené škály jsou dostatečné, ale názor na dostatečnost dokumentace k zaznamenávání všech ošetřovatelských problémů pacientů starších 65 let je rozdílný. Závěr: Úroveň kvality elektronicky vedené ošetřovatelské dokumentace u pacientů starších 65 let ve vybraném zdravotnickém zařízení odpovídá aktuálně zavedeným standardům, a tato dokumentace jako celek reflektuje z pohledu sester reálný stav potřeb pacientů starších 65 let.

Abstract:

Aim: The aim of the master thesis is to find out, using an audit investigation, whether the level of quality of electronically managed nursing documentation of patients older than 65 years corresponds to the currently established standards in the selected medical facility, and whether the electronic nursing documentation as a whole reflects the real state of needs of patients older than 65 years from the nurses' point of view. Methodology: The audit was conducted by the method of inspection of closed comprehensive nursing documentation at the following workplaces: Aftercare, Internal Medicine and Cardiology, Cardiac Surgery, Rehabilitation, Urology. An audit checklist was created with 75 criteria covering most of the recording options. 100 documentations of patients older than 65 years was audited. The audit was supplemented by the quantitative research conducted by a non-standardized anonymous questionnaire, created on the basis of the specificity of records of patients older than 65 years, intended for nurses working at audited workplaces. The research group consisted of 116 respondents. Both investigations took place from December 2021 to January 2022. Results: The audit found that at the workplaces there is a difference between the quality of documentation management of patients older than 65 years and patients of all ages. The quality of management varies between workplaces and decreases with increasing length of hospitalization. The questionnaire research found that between the workplaces, there is no difference in the nurses' opinion on the appropriateness of adding items to the electronic nursing documentation of patients older than 65 years. The offered scales are sufficient, but the opinion on the sufficiency of documentation to record all nursing problems of patients older than 65 years varies. Conclusion: The level of quality of electronically managed nursing documentation of patients older than 65 years in the selected medical facility corresponds to the currently established standards, and the documentation as a whole reflects, from the nurses' point of view, the real state of needs of patients older than 65 years.
