Molecular typing of multidrug resistant bacteria causing infections in patients with hematological malignancies

Abstract:

Úvod: Závažnou komplikací léčby, u imunokompromitovaných pacientů často život ohrožující, jsou bakteriální infekce. Správné a rychlé určení původce infekce a jeho typizace je proto zásadní pro rychlé protiepidemické opatření a zahájení cílené terapie. Cíle: Cílem disertační práce bylo optimalizovat rychlé a cenově dostupné molekulárně typizační metody využitelné pro prospektivní typizování bakterií v klinické praxi. Těmito metodami zhodnotit vztah kolonizačních a invazivních kmenů a využít je pro detekci outbreaků. Metody: Pro rychlou typizaci bakterií byly navrženy metody Mini-MLST a rep-PCR, kterými byly otestovány klinické izoláty Klebsiella pneumoniae s produkcí širokospektré beta-laktamázy (ESBL KLPN) a citlivé kmeny Escherichia coli. Vybrané izoláty ESBL KLPN byly charakterizovány pomocí celogenomového sekvenování. Výsledky: V rámci řešení této práce bylo ověřeno, že zvolené metody jsou vhodné pro prospektivní monitorování epidemiologické situace. Metodami samostatně nebo jejich kombinací byly odhaleny outbreaky mezi pacienty a objasněn vztah kolonizačních a invazivních kmenů. Celogenomovým sekvenováním bylo identifikováno velké množství genů antibiotické rezistence, faktorů virulence, plazmidů a celosvětově rozšířených i sporadicky se vyskytujících sekvenčních typů. Součástí této disertační práce je také přehled o procentuálním zastoupení multirezistentních kmenů bakterií a jejich podílu na infekcích krevního řečiště u imunokompromitovaných nemocných. Závěr: Byly splněny vytčené cíle disertační práce, získané výsledky pomohly objasnit příbuznost získaných klinických izolátů bakterií a strukturu populace ESBL KLPN ve Fakultní nemocnici Brno. Zavedené metody slouží k prospektivnímu testování bakteriálních izolátů a výsledky jsou využívány k prospektivnímu sledování epidemiologické situace. Spektrum testovaných bakterií se postupně rozšiřuje o další klinicky významné patogeny.

Abstract:

Introduction: Bacterial infections pose a significant and, as is frequently the case, life-endangering threat in treatment of immunocompromised patients. Therefore, it is imperative that the cause of such an infection be determined as well as typed correctly and expeditiously so as to ensure quick anti-epidemic measures are taken and targeted therapy is initiated. Goals: This thesis seeks to optimize, on a routine basis, expeditious and affordable molecular typing methods that may be used to type prospectively bacteria in clinical practice. These methods are utilized to evaluate the relationships between colonizing and invasive strains as well as detect outbreaks. Methods: The thesis proposes that Mini-MLST and rep-PCR methods be used to perform expeditious bacteria typing; both methods have been applied to test clinical Klebsiella pneumoniae (KLPN) strains producing extended spectrum beta-lactamase (ESBL) as well as sensitive strains of Escherichia coli. Whole-genome sequencing has been employed to characterize selected ESBL KLPN strains. Results: The thesis provides evidence as to the assumption that the selected methods are suitable to monitor, prospectively, epidemiological situations. The methods in question have, separately and in combination, detected outbreaks in patients as well explained relationships between colonizing and invasive strains. Also, whole-genome sequencing identified a considerable number of antibiotic resistance genes, virulence factors as well as plasmids and rare and widespread sequence types. Additionally, the thesis includes an overview of percentages of multiresistant strains of bacteria as well as of how these contribute to bloodstream infections in immunocompromised patients. Conclusion: All the goals hereof have been achieved and the results obtained have helped to explain the relationships between clinical bacteria isolates and the structure of the ESBL KLPN populations in the University Hospital Brno. The methods introduced are used to test prospectively bacteria strains, and the results are used to perform epidemiological surveillance. Other clinically significant pathogens are constantly being added to the set of bacteria undergoing testing.
