Rehabilitation with the syndrome of painful shoulder

Abstract:

Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u pacientky s dg. syndrom bolestivého ramene volbou optimálních fyzioterapeutických postupů, konceptů a metod. Metodika: V této části je zpracována kazuistika ženy (46 let) s dg. sy bolestivého ramene. Na základě vstupního kinezilogického rozboru byl sestaven krátkodobý fyzioterapeutický plán. Fyzioterapeutická intervence probíhala v období jednoho měsíce formou 10 návštěv s využitím různých konceptů a metod myofasciální a mobilizační techniky, dechová cvičení, Brügger koncept, PNF (Proprioceptivní neuromuskulární facilitace), ACT (Akrální koaktivační terapie), DNS (Dynamická neuromuskulární facilitace), prvky Senzomotoriky, SM systém. V polovině terapie bylo provedeno kontrolní vyšetření zaměřené na rozsah pohybu a na konci výstupní fyzioterapeutický rozbor, na jehož základě byl sestaven dlouhodobý fyzioterapeutický plán včetně režimových opatření. Výsledky: u pacientky došlo ke zklidnění klidových bolestí, zlepšení rozsahu pohybu pletence ramenního i hrudní páteře a zmírnění bolestivosti při pohybu v terminálních polohách. V důsledku aktivace HSS (hluboký stabilizační systém) došlo ke zlepšení funkce pánevního dna a stabilizaci lopatek. Závěr: Práce mne obohatila o nové poznatky, pomohla mi pochopit úzkou souvislost mezi funkčním a strukturálním postižením pletence ramenního, a stala se přínosem pro moji další praxi. Pro pacientku se stal pozitivní efekt terapie impulsem pro úpravu životního stylu.

Abstract:

Aim: The aim of this bachelor thesis is to contribute to the issue of physiotherapy at a patient with painful shoulder syndrome using optimal physiotherapeutic procedure, concepts and methods. Methodology: In the main part there is a processed case report of a woman (46 years old) with the painful shoulder syndrome. Based on the initial kinesiological analysis, the pacient was assembled with a short-term rehabilitation plan. The physiotherapeutic intervention took one month in form of ten visits with the opportunity to use many different concepts and methods - myofascial and mobilization technology, breathing exercises, Brügger's concept, PNF (Proprioceptive Neuromuscular Facilitation), ACT (Acral Coactivation Therapy), DNS (Dynamic Neuromuscular Stabilization), elements of sensomotorics, SM system. In the middle of the therapy a control examination focused on the range of motion was performed and at the end a final physiotherapeutic analysis was performed, based on which a long-term physiotherapeutic plan was compiled including some regime steps. Results: The patient has been relieved of restless pain, improving the extent of the shoulder girdle movement and thoracic spine, and a pain relief while moving in the terminal positions. As a result of the activation of the HSS (deep stabilization system), the pelvic floor function improved and the blades were stabilized. Conclusion: The thesis has enriched me with new knowledge, helped me understand the close link between the functional and structural affection of the shoulder girdle and became an acquisition for my next practice. For the patient there has been positive effect of the therapy and this is an impulse to change her lifestyle.
