Nursing process in a care of a patient with gallbladder and biliary tract disease

Abstract:

Bakalářská práce se zabývá onemocněním žlučníku a žlučových cest. Teoretická část se soustřeďuje na popis vybraných onemocnění žlučníku a žlučových cest a specifiky ošetřovatelské péče. Praktická část tato specifika podrobně popisuje a zahrnuje hlavní cíl, který představuje ošetřovatelský proces pacienta včetně stanovení ošetřovatelské diagnózy, určení ošetřovatelského cíle, vypracování ošetřovatelských intervencí s jejich následným vyhodnocením. Došlo ke splnění hlavního cíle bakalářské práce, přičemž vedlejší cíle v rámci jednotlivých částí ošetřovatelské péče byly plně nebo částečně splněny. Sběr dat se uskutečnil formou cíleného rozhovoru s vybraným pacientem, komunikací se zdravotnickým personálem a nahlížením do dokumentace. Rozhovor s pacientem proběhl dle struktury 13 domén NANDA-I Taxonomie II, který sloužil ke zpracování ošetřovatelské anamnézy. Pro získání potřebných informací bylo součástí péče také fyzikální vyšetření sestrou a využití specializovaných škál a testů pro objektivní posouzení stavu pacienta - GCS (stav vědomí), VAS (využito pro míru bolesti), stupnice dle Nortonové (dekubity), APAIS a STAI (úzkost), stupnice tíže flebitis dle VIP, Barthelův test základních všedních činností a riziko pádu dle Morse. Praktická část se dále věnuje popisu vývoje onemocnění během hospitalizace pacienta zahrnující ošetřovatelské intervence autorky bakalářské práce včetně edukace a monitorace stavu pacienta.

Abstract:

The bachelor thesis is specfically concerned with diseases of the gallbladder and bile ducts. The theoretical part focuses on describing selected diseases of the gallbladder and bile ducts and the specifics of nursing care. The practical part extensively describes these specifics and includes the main objective, which is the nursing process of the patient, including determining nursing diagnosis, setting nursing goals, developing nursing interventions with subsequent evaluation. The main aim of the bachelor's thesis was met, while the secondary aims within the individual parts of nursing care were fully or partially met. Data collection was conducted through targeted interview with selected patient, communication with healthcare personnel, and review of documentation. The patient interview followed the structure of the 13 domains of NANDA-I Taxonomy II, which served for processing nursing history. To obtain necessary information, physical examination by the nurse and the use of specialized scales for objective assessment of the patient's condition - GCS, VAS, Norton's scale, APAIS, STAI and Maddona's phlebitis severity scale were also part of the care. The practical part further describes the development of the disease during the patient's hospitalization, including nursing interventions by the author of the bachelor thesis, as well as education and monitoring of the patient's condition.
