Retrospective analysis of partial and weak RHD molecular typing in DNA laboratory of Blood Center

Abstract:

Tato práce měla za cíl retrospektivní analýzu pacientů, u kterých bylo indikováno vyšetření slabého a variantního RHD pomocí analýzy DNA metodou PCR-SSP, především kvůli slabým anebo nejasným sérologickým výsledkům. Analýza byla zaměřena na D antigen krevního Rh systému, záměrně pro jeho známou vysokou imunogenicitu a tedy vysokou klinickou významnost. Jednoznačné stanovení D antigenu a přesná specifikace jeho atypických forem je důležité zejména z hlediska prevence a léčby hemolytického onemocnění novorozence, adekvátní aplikace imunoprofylaxe u gravidních, a pro optimalizaci transfuzní léčby. Analyzovaný soubor zahrnoval 97 pacientů, z toho se podařilo molekulárně genetickým vyšetřením identifikovat atypickou formu D antigenu u 84,5 % (82/97), u zbylých osob nebyla atypická forma D antigenu prokázána. Ve vyšetřovaném souboru převažovaly ženy tj. 82 % (80/97) s diagnózou související s těhotenstvím, porodem či potratem tj. 55,7 % (54/97). Zjištěné četnosti jednotlivých atypických forem D antigenu více méně odpovídaly těm, které byly pro populaci ČR publikovány již dříve. Nejčastěji detekovaným typem byl slabý D typ 1. Genotypizace krevního Rh systému pomocí metody PCR-SSP je vhodnou volbou v případě řešení nejasných sérologických výsledků. Jde o metodu technicky i finančně nenáročnou a tím pádem výhodnou z hlediska jednoznačné identifikace běžných atypických forem D antigenu.

Abstract:

The aim of this bachelor thesis was to retrospectively analyse weak and partial RHD genotyping in DNA laboratory of Blood center at the University Hospital Ostrava. The analysis was focused on the RhD antigen of the Rh blood system because this antigen is known as the most immunogenic after AB0 system and so one of those most clinically significant. An accurate determination of the RhD antigen and its atypical forms is important in terms of prevention and treatment of hemolytic disease of the newborn, for the optimalisation of transfusion therapy and anti-D immunoprophylaxis. The analyzed file consisted of 97 patients, of which has been determined by molecular genetic testing atypical form of D antigen in 84,5 % (82/97) and for the remaining patients the atypical form of D antigen has not been proven. In the tested file predominated women 82 % (80/97) and 55,7 % (54/97) of them was proven diagnosis associated with pregnancy, childbirth or abortion. Detection frequency of the individual atypical forms of D antigen more or less corresponded to those which were published for czech population earlier. The weak D type 1 was the most often detected type. Genotyping of the weak a partial RHD using PCR-SSP can be a proper option for the unambiguous assignment of the clinically relevant atypical forms of RhD antigen, in case of inconsistent or weak serological typing results. The method is easy to implement and perform for staff and economically undemanding.
