Detection of T Wave Parameters from Multilead ECG Signals

Abstract:

Diplomová práce se zabývá metodami detekce parametrů signálu EKG se zaměřením na T vlnu a její morfologii. V teoretické části práce jsou podrobně popsány nejen základní parametry signálu EKG (vlna P, T, U, QRS komplex a příslušné intervaly), ale také specializované parametry morfologie T vlny (celkový kosinus úhlu QRS – T, reziduum T vlny, disperze morfologie T vlny a další). Ty se z pravidla vyznačují tím, že k jejich stanovení není potřeba problematická detekce začátku a konce T vlny v časové oblasti. Následně je zde popsán přínos analýzy morfologie T vlny z pohledu různých excitací hemodynamického stavu a diferenciace skupin zdravých dobrovolníků a pacientů s hypertenzí či metabolickým syndromem. V praktické části práce byl navržen poloautomatický detektor konce QRS komplexu (bod J), začátku T vlny, maxima T vlny a plochy T vlny v prostředí MATLAB. Pomocí těchto parametrů byla provedena detailní analýza signálu EKG se zaměřením na morfologii T vlny. Studován byl vztah délky všech dílčích úseků EKG a srdeční frekvence. K dispozici byla data zdravých dobrovolníků a pacientů se začínající hypertenzí nebo metabolickým syndromem ze zátěžového měření EKG (měřený subjekt cvičí na speciálním kole) a z měření EKG při naklánění lůžka. V některých případech se jednalo o vícesvodový záznam EKG. Kvalita detekce navrženými algoritmy byla hodnocena zvlášť pro skupiny s různým typem excitace a zdravotním stavem. Následná analýza vlivu zrychlení srdeční frekvence na délku jednotlivých úseků EKG, změn velikosti amplitudy a obsahu plochy T vlny byla také provedena zvlášť pro jednotlivé skupiny měřených subjektů. Vliv onemocnění a typu excitace byl statisticky testován pomocí programu STATISTICA.

Abstract:

The thesis deals with the methods of ECG signal parameters detection with the focus on the T wave and its morphology. In the theoretical part of the thesis, not only the basic parameters of the ECG signal (P, T, U wave, QRS complex and related intervals), but also the specialised parameters of the T wave morphology (total cosine of the QRS-T angle, relative T wave residuum, T-wave morphology dispersion at al.) are described in detail. In principle, they are characterised by a fact, that the problematic detection of the beginning and end of the T wave in a time domain is not needed. Then, the contribution of the T wave morphology analysis from the perspective of different excitations of the hemodynamic state and differentiation of the groups of healthy volunteers and patients with hypertension or metabolic syndrome is described. In the practical part of the thesis, a semiautomatic detector of the end of the QRS complex (J point), the beginning, the maximum and the surface of the T wave has been designed in the MATLAB environment. With the help of these parameters, a detailed analysis of the ECG signal with the focus on the T wave morphology has been carried out. The relation of the length of all partial segments of the ECG and the heart rate has been studied. Available data of healthy volunteers and patients with early hypertension or metabolic syndrome from the exercise ECG measurement (the measured subject is riding a special bike) and from the ECG measurement with a tilting bed has been used. In some cases, multi-lead ECG record has been provided. The quality of the detection by the designed algorithms has been evaluated for individual groups with a different type of excitation and health state. Subsequent analysis of the influence of the heart rate quickening on the length of ECG segments, changes in the amplitude size and the surface of the T wave has been carried out for individual groups of measured subjects. The influence of the disease and the type of the excitation has been statistically tested with the STATISTICA software.
