The subjective experience of women after abortion

Abstract:

Cíl: Cílem diplomové práce bylo zjistit a popsat prožívání žen po samovolném potratu, identifikovat potřeby žen po samovolném potratu a způsoby zvládání zátěže a formulace doporučení týkající se zkvalitnění péče o tyto ženy pro porodní asistentky v ústavní i komunitní péči Metodika: Výzkumný soubor tvořilo 9 žen, které samovolně potratily v prvním trimestru gravidity. Po splnění nastavených kritérií následně proběhly polostrukturované rozhovory, které byly autenticky přepsány a analyzovány. Analýza směřovala k vytvoření jednotlivých kategorií a subkategorií. Výsledky: Analýzou rozhovorů bylo zjištěno, že období po spontánním potratu je obdobím zvýšené emocionální zátěže a že ženy používají různé způsoby jejího zvládání. Dále bylo zjištěno, že v oblasti potřeb dominuje potřeba získávání informací a potřeba sdílení pocitů. V oblasti podpory ženy spoléhaly pouze na laickou podporu, podpora z řad erudovaných odborníků byla nedostatečná. Závěr: Samovolný potrat je událost, která může způsobit duševní utrpení s potenciálně dlouhodobými důsledky. Psychologickým aspektům spontánního potratu věnuje odborná veřejnost jen malou pozornost a ženy po prodělaném samovolném potratu stojí na okraji zájmu nejen zdravotníků, ale celé společnosti. Špatná informovanost žen může vést ke zvýšenému výskytu úzkosti a negativních pocitů, které ovlivňují další spolupráci se zdravotníky. Pochopení pocitů a potřeb žen může být prvním krokem v procesu zlepšování a efektivitě poskytování ošetřovatelské péče. Pro tuto skupinu žen mohou být vhodnou a důležitou podporou dostatečně vzdělané porodní asistentky v komunitní péči a proto je vhodné podporovat vzdělávací programy v této oblasti. Zkvalitnění dalšího managementu péče o tyto ženy by měly přinést další studie.

Abstract:

Aim: The aim of this diploma thesis was to verify and describe experiences of women after misscarriage. Further, identify its needs and ways for solving of loading. Finaly, we tried to formulate recommendations related to improving quality of care about these women after misscarriage given for midwife in institutional and community care. Methods: Research sample was composed from 9 women, which had misscarriage in first trimestr of gravidity. With all women after passing defined critetions had been subsequently accomplished semistructured inteviews, which were afterthat autentic re-written and analysed. The analysis was directed for developing of defined categories and sub-categories. Results: The analysis of interviews found that time period after misscarriage represents the period of increase emotional loading and that women used different kinds for solving of this loading. In addition, it was revealed that in the area of needs dominate the need for getting information and the need for sharing feelings. In the area of support, women relied only on layman support. The support from the side of specialists was found as unsufficient. Conclusion: Misscarriage represents event which can be cause of mental suffering with potentional long-period implications. The academic public depute only a few attention to psychology aspects and women after misscarriage usually stay on periphery of interest at academic public as well as in society. Unsufficient amount of information for women after misscarriage can lead to increasing incidence of anxiety and negative feelings, which might influence further co-operation with medic. Understanding of feelings and needs of these women can represent the first step in the process of improving and increasing the efficiancy in providing of medical care. Suitable and important support for women after misscarriage could be provided by well educated midwifes in community care. Therefore, according our opinion its convinient to support edcuational programs in this area. Better quality of care should also bring further studies in this problematic.
