Total disc replacement by prosthesis Prodisc-L in lumbar degenerative disc disease

Abstract:

Disertační práce se soustředí na degenerativní onemocnění bederní páteře a meziobratlových plotének, které jsou nejčastější příčinou spondylochirurgické léčby vůbec. Zabývá se vznikem degenerativní diskopatie s rozvojem biochemických, histologických i anatomických změn plotének a jejich biomechanických důsledků na funkci postiženého páteřního segmentu. Rozborem klinických diskogenních syndromů a radiologických nálezů uvedených v práci lze stanovit optimální operační techniku zahrnující dekompresivní metody, fúze či dynamické stabilizace. V popředí chirugické léčby stojí dnes snaha o zachování fysiologické funkce páteřního segmentu s odstraněním patologické instability, dysfunkční ploténky či komprese nervových struktur. Tyto požadavky rekonstrukční chirurgie splňují totální náhrady plotének, které dosahují vynikajících klinických úspěchů při dodržení uváděných indikačních a grafických kritérii. Ve sledovaném souborů 50 nemocných po dobu více než dva roky od operace jsme zaznamenali 92% dobrých výsledků se signifikantním poklesem Oswestry i VAS skóre. V rámci experimentální studie srovnává teoretická část práce grafické nálezy na MRI se skutečným morfologickým stavem plotének zjištěným při sekci. Celkově jsme vyšetřili 30 plotének s osmi kadaverů a zjistili významnou signifikantní vazbu na stoupající stupně degenerace u těchto MRI parametrů hypointensita T2 signálu, Modicovy změny, osteofyty a změna tvaru nucleus pulposus. Disertační práce podporuje původní koncepci artroplastik a zdůrazňuje priority této poměrně nové chirurgické techniky v léčbě degenerativního onemocnění páteře. Disertační práce

Abstract:

The thesis focuses on degenerative lumbar and intervertebral disc disease, which is the most frequent reason for spine surgical treatment. It deals with the development of degenerative discopathy with regard to biochemical, histological and anatomical changes of discs and their biomechanical influence on the function of the affected spinal segment. Based on the analysis of clinical discogenic syndromes and radiological findings presented in the thesis, we can determine optimal surgical technique including decompressive methods, fusions or dynamic stabilizations. The key target for surgical treatment is to preserve the physiological function of spinal segments and to remove pathological instability, dysfunctional disc or compression of nerve structures. These prerequisites of reconstruction surgery are met by total disc replacement, which can achieve excellent clinical results when observing indications and radiological criteria. In the group of 50 patients followed up more than two years after the surgery we have observed 92% good results with a significant decrease in both Oswestry and VAS scores. The theoretical part of the experimental study compares MRI findings with actual morphological condition of discs harvested from cadavers. 30 discs from eight cadavers had been examined and we have observed significant relationship to increasing degeneration in the following MRI parameters T-2 intensity loss, Modic changes, osteophytes and changes in the shape of the nucleus pulposus. The thesis supports the original concept of arthroplasty and stresses priorities of this relatively new surgical technique in the treatment of degenerative lumbar disc disease.
