Characterization of antibiotic susceptibility of vaginal strains of Streptococcus agalactiae

Abstract:

Streptococcus agalactiae je obávaný patogen především u těhotných žen a novorozenců. U novorozených dětí se obáváme vzniku časné nebo pozdní neonatální sepse, u dospělých například infekce močových cest, komplikace těhotenství či základního onemocnění (diabetes mellitus, malignity a další). S. agalactiae je obvykle dobře citlivý na používaná antibiotika, mezi nejpoužívanější patří penicilin a ampicilin. Například u těhotných žen se antibiotika nasazují při porodu, jestliže tyto ženy měly pozitivní výsledek screeningu. Pokud nebylo screeningové vyšetření provedeno, nasazují se antibiotika podle určitých kritérií (například předčasný porod, předčasný odtok plodové vody, pozitivní kultivace moči v průběhu gravidity a další). V poslední době je však patrný nárůst rezistence k některým antibiotikům. Práce je proto zaměřená na vyhodnocení citlivosti/ rezistence S. agalactiae na antibiotika (penicilin, tetracyklin, vankomycin a nitrofurantoin). Rezistence na penicilin, vankomycin a nitrofurantoin nebyla přítomna ani u jednoho vyšetřovaného kmene S. agalactiae – penicilin můžeme potvrdit jako lék první volby. Rezistence byla přítomna pouze u tetracyklinu (66 % kmenů). Z porovnaných studií byla zjištěna zvyšující se rezistence kmenů na erytromycin a klindamycin.

Abstract:

Streptococcus agalactiae is a feared patogen especially for pregnant women or a newborn children. For a newborn children, we are affraid of early or late genesis of neonal sepsis. For adults, we are affraid of urinary infection, complications of pregnancy or of the basic diseases (diabetes melitus, malignity and more). S. agalictae is usually well susceptible to antibiotics that are used. The most frequently used antibiotics are penicillin and anpicillin. For example for pregnant women we prescribe antibiotics during the childbirth if the result of the screening was positive. If they had not carry out the screening test the antibitics are prescribe according to certain criteria (for example premature birth, premature draining amniotic, positive urine culture in the course of pregnancy and more). Recently, however, the apparent increase in resistance to certain antibiotics occurs. Our work is focused on the evaluation of the susceptibility/resistance S. agalactiae to antibiotics (penicillin, tetracycline, vancomycin and nitrofurantoin). The resistance to penicillin, vancomycin and nitrofurantoin was present in no tested strain of S. agalactiae – penicillin can be confirmed as the drug of first choice. Resistance was present only on the tetracycline (66 % of strains). From the other studies there has been found an increasing resistance of the strains to erythromycin and clindamycin.
