Regulace Th1 odpovědi v CD4 T-lymfocytech u pacientů s alergickým eosinofilním astmatem

Regulation of Th1 response in CD4 T-cells of patients with allergic eosinophilic asthma

Abstract:

Diplomová práce se zabývá možnostmi měření intracelulárního komplementu, tzv. komplosomu, který u CD4+ T-lymfocytů zajišťuje regulaci imunitní odpovědi. Kom-plosom zahrnuje především fragmenty C3a a C3b, které se váží na specifické kom-plementové receptory C3aR a CD46. Komplosom zajišťuje přežití klidového Th0-lymfocytu ve fázi homeostázy. V efektorové fázi zprostředkovává produkci IFN-γ a IL-2 aktivovanými Th1-lymfocyty. Ve fázi regulace ústí signalizace přes CD46 v pro-dukci IL-10 Tr1-lymfocyty a díky tomu k útlumu zánětlivé odpovědi. Deregulace systému komplosomu a jeho zvýšená aktivita vedou ke snížení pro-dukce IL-10. Tento stav ústí v patologickou hyperaktivní Th1 odpověď a byl popsán např. u autoimunitní artritidy nebo roztroušené sklerózy. Výsledky studií zkoumají-cích komplosom naznačují, že je při astmatu poškozen přesmyk Th1-lymfocytů na Tr1-lymfocyty při kostimulaci přes CD46 podobně jako u autoimunitních onemocně-ní. Cílem práce bylo zavedení metod ELISPOT A FLUOROSPOT k měření hladin cy-tokinů a následná analýza aktivace komplosomu CD4+ T-lymfocytů kontrolních osob a pacientů trpících astmatem. Ve vzorcích bez stimulace a se stimulací pomocí anti-CD3 a anti-CD46 se současným přídavkem IL-2 se měřila produkce IFN-γ a IL-10 me-todami ELISPOT, FLUOROSPOT, ELISA a exprese C3b, C3aR a CD46 pomocí průtoko-vé cytometrie. U kontrolních osob se při uvedené stimulaci podařilo aktivovat systém komplo-somu. U astmatiků byla potvrzena aktivace komplosomu v CD4+ T-lymfocytech peri-ferní krve působením alergického zánětu. Byly úspěšně zavedeny a ověřeny metody ELISPOT a FLUOROSPOT. Vypracované postupy obou metod byly využity k vyhodnocení produkce zkoumaných cytokinů po aktivaci komplosomu CD4+ T-lymfocytů u kontrolních osob a astmatiků. Získané výsledky ukázaly, že v produkci cytokinů řízené komplosomem nebyl mezi zkoumanými skupinami kontrol a pacien-tů nalezen rozdíl. Zahraniční studie však uvádějí, že při astmatu dochází ke změnám fungování systému komplosomu. Především popisují sníženou produkci IL-10 u astmatiků po stimulaci. Je pravděpodobné, že naše výsledky byly ovlivněny nízkým počtem zkoumaných osob: 12 vzorků kontrolních osob a 8 vzorků pacientů. Pro ově-ření výsledků by bylo vhodné provést rozsáhlejší studii, která by zahrnovala větší soubory dat.

Abstract:

The diploma thesis deals with the possibilities of measuring intracellular comple-ment, so-called the complosome, which ensures the regulation of the immune re-sponse in CD4+ T-lymphocytes. The complosome mainly includes fragments C3a and C3b, which bind to specific receptors C3aR and CD46. The complosome ensures the survival of the resting Th0-lymphocyte in the phase of homeostasis. In the effector phase, it mediates the production of IFN-γ and IL-2 by activating Th1-lymphocytes. In the regulation phase, signaling through CD46 results in the production of IL-10 by Tr1-lymphocytes. This switching to IL-10 production ensures an early suppression of the inflammatory response. Deregulation of the complosome and its increased activity lead to a decrease in IL-10 production. This condition results in a pathological hyperactive Th1 response and has been described, for example, in autoimmune arthritis or multiple sclerosis. The results of studies investigating the complosome indicate that in asthma the switch from Th1-lymphocytes to Tr1-lymphocytes is damaged during costimulation via CD46, similar to autoimmune diseases. The goal of this work was the introduction of ELISPOT and FLUOROSPOT meth-ods to measure cytokine levels and subsequent analysis of CD4+ T-lymphocyte com-plosome activation of control subjects and asthma patients. In unstimulated samples and samples stimulated by anti-CD3 anti-CD46 and IL-2, the production of IFN-γ and IL-10 was measured by ELISPOT, FLUOROSPOT, and ELISA. The expression of C3b, C3aR, and CD46 was measured using flow cytometry. In the control subjects, the complosome was activated by the said stimulation. Activation of the complosome in peripheral blood CD4+ T-lymphocytes by allergic inflammation was found in asthmatics. The ELISPOT and FLUOROSPOT methods were successfully introduced and validated. The developed procedures of both meth-ods were used to evaluate the production of the investigated cytokines after the acti-vation of the complosome of CD4+ T-lymphocytes in control subjects and asthmatics. The obtained results showed no differencein cytokine production between the exam-ined groups of controls and patients. However, foreign studies report that there are changes in the functioning of the complosome in asthma (especially reduced produc-tion of IL-10). Our results were likely influenced by the low number of subjects: 12 control subjects and 8 patients. To verify the results, conducting a more extensive study on larger data sets would be appropriate.
