Automated external defibrillator and its effective use with the help of first responders

Abstract:

Tato bakalářská práce se zabývá problematikou využití automatizovaných externích defibrilátorů (AED) v přednemocniční neodkladné péči, především v kontextu jejich použití v rámci systému first responderů. Cílem práce je zhodnotit efektivitu AED při zásazích mimo nemocniční prostředí, identifikovat jejich přínos pro přežití pacientů s náhlou zástavou oběhu a upozornit na případná omezení v reálných podmínkách. Teoretická část shrnuje historický vývoj, principy fungování AED, jejich typy, technické aspekty a situace, ve kterých může dojít k selhání zařízení nebo algoritmu. Zvláštní pozornost je věnována také organizaci systému first responderů v jednotlivých krajích České republiky. Praktická část je postavena na kombinaci kvantitativní a kvalitativní analýzy. Kvantitativní analýza vychází z dat Zdravotnické záchranné služby Ústeckého kraje a hodnotí aktivace AED mezi lety 2011 a 2024. Kvalitativní část obsahuje kazuistiku dokumentující případ selhání AED při detekci defibrilovatelného rytmu, která byla podrobně analyzována včetně posouzení technického stavu konkrétního přístroje. Výsledky ukazují, že systém first responderů přispívá ke zkrácení doby do první defibrilace a výrazně zvyšuje šance na přežití. Současně práce poukazuje na důležitost technické spolehlivosti AED a nutnost lidského dohledu při jejich použití v terénu.

Abstract:

This bachelor thesis deals with the use of automated external defibrillators (AEDs) in pre-hospital emergency care, especially in the context of their use within the first responder system. The aim of the thesis is to evaluate the effectiveness of AEDs in out-of-hospital interventions, to identify their benefit for the survival of patients with sudden circulatory arrest and to highlight potential limitations in real-life settings. The theoretical part summarizes the historical development, principles of AEDs, their types, technical aspects and situations in which the device or algorithm may fail. Particular attention is also paid to the organisation of the first responder system in individual regions of the Czech Republic. The practical part is based on a combination of quantitative and qualitative analysis. The quantitative analysis is based on the data of the Medical Rescue Service of the Ústí nad Labem Region and evaluates AED activations between 2011 and 2024. The qualitative part includes a case report documenting a case of AED failure in detecting a defibrillatable rhythm, which was analysed in detail including an assessment of the technical condition of the specific device. The results show that the functionality of the first responder system, within the available data, appears to be an effective mechanism to ensure timely AED use and survival of patients with sudden circulatory arrest. At the same time, the work highlights the importance of technical reliability of AEDs and the necessity of human supervision during their use in the field.
