Physiotherapy after LCA knee injury

Abstract:

Úvod: Bakalářská práce je věnována problematice fyzioterapie po operativním řešení ruptury LCA kolenního kloubu u aktivního sportovce věnujícího se zejména fotbalu, a to jak v podobě hráče, tak sudího. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie po zranění LCA kolenního kloubu volbou vhodných fyzioterapeutických postupů, metod a konceptů. Metodika: Hlavní část práce je věnována kazuistice pacienta po operaci ruptury předního zkříženého vazu pravého kolenního kloubu. Celkově proběhlo devatenáct intervencí v průběhu osmi měsíců, v délce intervence od 50 do 90 minut. Součástí první intervence byl vstupní kineziologický rozbor se všemi náležitostmi, na základě kterého byl vytvořen krátkodobý fyzioterapeutický plán. Při páté a čtrnácté intervenci byly provedeny kontrolní kineziologické rozbory, na základě kterých byl vždy upraven plán fyzioterapie. Výstupní kineziologický rozbor byl proveden na poslední devatenácté intervenci spolu se se stavením dlouhodobého fyzioterapeutického plánu. Během terapií byly využity např. dynamická neuromuskulární stabilizace, akrální koaktivační terapie, senzomotorická stimulace na labilních plochách. Výsledky: V průběhu naší spolupráce se podařilo zvětšit pacientův rozsah pohybu do flexe pravého kolenního kloubu z 60° pasivně naměřeného při vstupním kineziologickém rozboru na 125° aktivně naměřeného při výstupním rozboru. Dále zvětšení svalové síly ze stupně tři do flexe a extenze pravého kolenního kloubu na výsledný stupeň pět, což podkládá porovnání obvodu končetiny měřeného 10 cm nad patellou mezi prvním kontrolním (46, 5 cm) a výstupním kineziologickým rozbor (51, 5 cm). A v neposlední řadě se uskutečnil návrat pacienta do sportovních aktivit, které byl zvyklý vykonávat před zraněním. Závěr: Pozitivní výsledek byl dosažen zejména z důvodu disciplinovanosti pacienta k autoterapiím. Obecně si naší spolupráce vážím a věřím, že mě obohatila o cenné zkušenosti, které využiji v budoucích intervencích s jinými pacienty.

Abstract:

Introduction: This Bachelor thesis is focused on the problem of physiotherapy after surgical repair of ACL knee rupture in an active athlete involved mainly in football, both as a player and a referee. Aim: The objective of this thesis is to contribute to solving the problem of physiotherapy after ACL knee injury by choosing appropriate physiotherapeutic procedures, methods and concepts. Methodology: The main part of the thesis is devoted to a casualty of a patient after anterior cruciate ligament rupture of the right knee joint. A total of nineteen interventions were performed over a period of eight months, ranging in length from 50 to 90 minutes. The first intervention included an initial kinesiological analysis with all the relevant details, based on which a short-term physiotherapy plan was created. At the fifth and fourteenth intervention, control kinesiological analyses were performed, based on which the physiotherapy plan was always modified. The final kinesiological analysis was performed at the last nineteenth intervention together with the construction of a long-term physiotherapy plan. The therapies included dynamic neuromuscular stabilization, acral coactivation therapy, sensorimotor stimulation on labile surfaces. Results: During our cooperation, we were able to increase the patient's range of motion into flexion of the right knee joint from 60° passively measured at the initial kinesiological analysis to 125° actively measured at the final analysis. Furthermore, an increase in muscle strength from grade three to flexion and extension of the right knee joint to a final grade five, which is supported by a comparison of limb circumference measured 10 cm above the patella between the first control (46.5 cm) and the final kinesiological analysis (51.5 cm). Last but not least, the patient was able to return to the sporting activities he was used to before the injury. Conclusion: The positive result was achieved mainly because of the patient's discipline to the self-therapy. In general, I appreciate our collaboration and believe that it has enriched me with valuable experience that I will use in future interventions with other patients.
