Abstract:

Tato diplomová práce je retrospektivní studie týkající se lipoproteinu Lp (a) a homocysteinu. Lipoprotein (a) je lipoproteinová částice, obsahující apolipoprotein (a) a apolipoprotein B-100. V odborné literatuře je zahrnutý do nezávislých faktorů aterosklerózy spolu s homocysteinem. Homocystein je aminokyselina s obsahem síry. Zaměřili jsme diplomovou práci na využívání stanovení Lp(a) a homocysteinu klinickými pracovníky. Posuzujeme počet vyšetření lipoproteinu Lp(a) a homocysteinu za posledních šest let v laboratoři Ústavu klinické biochemie FN Ostrava. Lp(a) je často stanovován v kombinaci s homocysteinem (dalším nezávislým rizikovým faktorem aterosklerózy). Cílem této práce je posoudit vzájemné vztahy těchto dvou faktorů. Vzorky byly vyšetřeny v laboratoři Ústavu klinické biochemie FN Ostrava v letech 2002 až 2007. Lipoprotein (a) je v Ústavu klinické biochemie vyšetřován od roku 1998, ale rutinně je vyšetřován od roku 2002. Lipoprotein (a) byl kvantifikován pomocí nefelometrie a homocystein byl stanoven pomocí plynové chromatografie. Celkem jsme vyhodnotili stanovení u 1246 pacientů. V průběhu sledovaného období (2002 ? 2007) počet vyšetření stoupal až do roku 2005, pak byl zaznamenán trvalý pokles. Nejčastěji uváděnými diagnózami byly vada koagulace (D68.9) a cévní mozková příhoda (I64).

Abstract:

The theses is a retrospective study dealing with lipoprotein (Lp) and homocysteine screening. Lipoprotein molecule comprises of apolipoprotein (a) and apolipoprotein B-100. In the literature is Lp(a) considered to be an independent factor of atherosclerosis, together with homocysteine. We concentrated on determination of Lp(a) and homocysteine by clinical workers. We examined the number of investigations of Lp(a) and homocysteine during the last six years in the Laboratory of Institute of Clinical Biochemistry at Faculty Hospital in Ostrava. Lp(a) examination proceeds mostly in combination with homocysteine. The aim of the Thesis is to evaluate mutual relations between the two factors. Samples were analyzed in the aforementioned laboratory within years 2002-2007. The lipoprotein has been determined by means of nephelometry, the homocysteine using gas chromatography. Within the given period, 1246 patients in total were controlled for the screening. Most of the analyses was done till 2004, later the number of investigations declined. Among the most often mentioned diagnoses was that of D68.9 (defect in coagulation), and I64 (vascular encephalic event).
