Vliv abúzu psychoaktivních látek na počet relapsů a souvislost se změnou objemů hippokampů u pacientů se schizofrenií

An impact of psychoactive substance abuse on a number of relapses and its connection to hippocampal volume change in patients with schizophrenia

Abstract:

Úvod. Průběh schizofrenie je ovlivněn několika známými proměnnými. Jejich vliv je modulován interakcí s genetickou dispozicí jednotlivce. Jedním z klinicky nejvýznamnějších a nejlépe ovlivnitelných faktorů je abúzus psychoaktivních látek, zejména kanabisu. Psychotická epizoda má negativní vliv na další průběh onemocnění a působí také strukturální změny v CNS, což je prokázáno ve studiích s délkou neléčené psychózy. Jednou z nejčastěji studovaných struktur na mozcích pacientů se schizofrenií je hippokampus, který vykazuje dynamické změny objemu v průběhu onemocnění. Úbytek objemu byl pozorován jak u pacientů s první epizodou, tak u chronického onemocnění a byl spojen s nepříznivým průběhem nemoci. Dynamika těchto změn však zůstává nejasná. Metody. I. Průřezová studie 24 pacientů se schizofrenií a 24 zdravých dobrovolníků s použitím VBM analýzy sledovala vztah mezi hippokampálním GMV a trváním nemoci, počtem psychotických epizod a věkem. II. Longitudinální 4-letá prospektivní studie 58 pacientů s FES. Sledovali jsme délku trvání neléčené psychózy, počet psychotických epizod a reziduální příznaky. Objemy hippokampů byly měřeny pomocí ABM i VBM analýzy na počátku onemocnění. III. Průřezová studie sledovala u 105 ambulantních pacientů se schizofrenií vztah mezi přítomností abúzu a průběhem schizofrenního onemocnění včetně funkčních a sociálních schopností. Výsledky. I. Práce prokázala negativní korelaci počtu psychotických epizod s GMV levého hippokampu. Nebyla nalezena korelace s trváním nemoci. II. Redukce objemu hippokampu byla přítomna už v čase první epizody a její velikost byla ve vztahu k délce neléčené psychózy. Nenalezli jsme vztah objemů hippokampů vůči následnému (příznivému vs. nepříznivému) průběhu. III. Nalezli jsme signifikantní vztah mezi přítomností současného abúzu kanabinoidů a amfetaminů a průměrným počtem relapsů za rok. Abúzus neměl negativní dopad na sociální zapojení ani funkční schopnosti pacientů. Závěr. Prokázali jsme vliv počtu psychotických epizod na redukci objemů hippokampů bez ohledu na délku trvání nemoci a také podobný negativní vliv délky neléčené psychózy, což ukazuje na klinicky významný toxický vliv času stráveného v psychóze na mozkovou tkáň. V longitudinální studii jsme vyloučili možnost premorbidních změn u pacientů s následně nepříznivým průběhem. Zdá se, že právě kombinace kanabisu a amfetaminů je zvláště nepříznivá pro nárůst frekvence psychotických epizod. Vliv abúzu na funkční a sociální aspekty není průkazný a ukazuje na možnost obrácené kauzality.

Abstract:

Background. The course of schizophrenia is influenced by a few of the known variables. Their impact is modulated by the interactions with one´s genetic disposition. Psychoactive substance abuse, particularly cannabis, is one of the most important and the best influenceable factors. Psychotic episode exerts a negative impact on further course of the illness and it causes some structural changes in CNS as well. This was established in the studies focused on duration of untreated psychosis. Hippocampus is one of the most often studied regions in brains of patients with schizophrenia. It shows dynamic volume changes during the course of the illness. A volume decline was observed both in patients with first episode and in chronic illness and it was connected to less favourable illness course. However, the dynamics of such changes remain unclear. Methods. I. A cross-sectional study with 24 patients with schizophrenia and 24 healthy controls using VMB analysis followed a relation between hippocampal GMV and duration of illness, number of psychotic episodes and age. II. A longitudinal 4-years prospective study with 58 patients with FES. We followed a duration of untreated psychosis, number of psychotic episodes and residual symptoms. Hippocampal volumes were measured both with ABM and VBM analysis at baseline. III. A cross sectional study followed 105 outpatients with schizophrenia and analyzed a relation between a presence of substance abuse and a course of schizophrenia including functional and social abilities. Results. I. The study demonstrated a negative correlation between the number of psychotic episodes and left hippocampal GMV. There was no correlation with the duration of the illness. II. The reduction of the hippocampal volume was present at the time of first episode and its magnitude correlated wit0068 the duration of untreated psychosis. We found no relation of the hippocampal volume to further (more or less favourable) course. III. We found significant relation between current cannabinoid and amphetamine abuse and an average number of relapses per year. Substance abuse showed no negative impact on social adjustment or functional abilities of the patients. Conclusion. We proved the impact of a number of psychotic episodes on hippocampal volume reduction regardless duration of illness and similar negative impact of the duration of untreated psychosis, what suggests clinically important toxic influence of the time spent in psychosis on a brain tissue. We excluded a possibility of premorbid changes in patients with further unfavourable course in the longitudinal study. Just the combination of cannabis and amphetamines seems to have particularly adverse impact on the increase of frequence of psychotic episodes. The impact of substance abuse on functional and social aspects is not conclusive and it suggest rather the possibility of reverse causality.
