Prenatal diagnosis: current options and trends

Abstract:

Bakalářská práce se zabývá formou rešerše odborné literatury Prenatální diagnostikou: současnými možnostmi a trendy. Prenatální diagnostika má v současné medicíně a lékařské praxi stále větší význam. Řada závažných onemocnění nebo vrozených vývojových vad je spojena s chromosomovými aberacemi. Prenatální diagnostika je zaměřena z velké části právě na detekci těchto chromosomových změn. Postupy a metody, na které se ve své práci zaměřuji, se dělí z hlediska plodu na invazivní, do kterých se řadí zejména amniocentéza, biopsie choria, kordocentéza a neinvazivní metody, kam patří ultrazvukové vyšetření a biochemické vyšetření z krve matky. Cílem této bakalářské práce je podat odborné veřejnosti přehled současného stavu celosvětové diskuze o tom, který screeningové program se jeví jako nejefektivnější. Počáteční kapitoly obecně charakterizují prenatální diagnostiku, vrozené vývojové vady a screening vrozených vývojových vad a hlavní screeningové strategie. Ve stěžejní části bakalářské práce jsou diskutovány screeningové programy prvního trimestru (kombinovaný test s dalšími ultrazvukovými markery), druhého trimestru (triple test) i programy kombinující první a druhý trimestr (integrovaný test, sérum integrovaný test) a cell free DNA testy. V dalších částech této práce se zabývám invazivními přístupy k prenatálnímu sceeningu, prenatální genetickou diagnostikou, která zahrnuje cytogenetické, molekulárně cytogenetické a molekulárně genetické metody. Poslední kapitola obsahuje grafické znázornění vývoje prenatální diagnostiky v České republice.

Abstract:

The bachelor thesis deals with Prenatal diagnosis: current options and trends in a form of literary research. Prenatal diagnostics has greater importance in current medicine and medical practice. Many serious disorders and congenital defects are connected with chromosomal aberrations. Prenatal diagnostics detects these chomosomal aberrations. Procedures and methods on which I focus in my thesis are divided on invasive (there are amniocentesis, chorion biopsy, cordocentesis) and non-invasive methods (ultrasound examination and mother´s blood biochemical examination). The aim of this bachelor thesis is to give an overview of current status of discussion of effectivness of prenatal screeening programs. The initial chapters describe in general prenatal diagnosis, congenital defects, noninvasive methods of screening of congenital defects and structure of basic screening strategies. In the main part noninvasive screening tests of the first trimester (combined test with or without new sonographic markers), the second trimester (triple test) and tests which combine markes of the first trimester with markers of the second trimester (integrated test, serum intergrated) and the cell free DNA tests are discussed. In the other parts of this thesis I deal with invasive approach to prenatal sceening, prenatal genetic diagnosis that includes cytogenetic, molecular cytogenetic and molecular genetic methods. Last chapter contains graphical representation of development of prenatal diagnosis in the Czech Republic.
