Genetic factors in Breast cancer

Abstract:

Úvod: Karcinom (ca) prsu je v současnosti jedním z nejvíce probíraných a také nejčastěji se vyskytujících nádorových onemocnění u žen. V České republice se ročně diagnostikuje až 7200 nových případů a incidence se v čase stále zvyšuje. Mezi rizikové faktory, které se podílejí na vzniku ca prsu, patří faktory hormonální, nutriční, zevního prostředí a také faktory genetické. Geny, které jsou nejvíce asociovány s ca prsu, ale také s ca ovaria, nazýváme geny BRCA1 a BRCA2. Dle českých statistik se celoživotní kumulativní riziko vzniku ca prsu u žen s potvrzenou mutací v genech BRCA1 nebo BRCA2 pohybuje v rozmezí 55-85 %. Cíl: Cílem této studie bylo na základě molekulárně genetické analýzy zjistit přítomnost patogenní sekvenční varianty v genech BRCA1, BRCA2, popřípadě jiných genů, zjistit četnost výskytu mutací a procentuální podíl nemocných žen v korelaci s přítomností patogenní sekvenční varianty. V neposlední řadě na výsledek navázat také odpovídající preventivní péči. Metodika: S vývojem laboratorní diagnostiky se nyní běžně k detekci mutací používají metody masivního paralelního sekvenování (NGS), které umožňují detekci také jiných patogenních sekvenčních variant nebo variant nejasného významu, které nebyly dříve detekovány. Ve studii byly využity různé metody laboratorní analýz u 2033 žen převážně z Moravskoslezského kraje. Výsledky: Do molekulárně genetické analýzy bylo zařazeno 2033 českých žen a zachyceno 212 patogenních mutací. 127 žen bylo pozitivních v genu BRCA1 (60 %), 72 žen v genu BRCA2 (34 %) a 13 žen v jiných kandidátních genech (CHEK2, PALB2, ERCC4) (6 %). Výsledky ukazují, že 74 % žen s potvrzenou mutací v genu BRCA1 a 83,3 % žen s BRCA2 již mělo ca prsu, 21,3 % s BRCA1 a 15,3 % s BRCA2 mělo ca vaječníku a 4,7 % žen s BRCA1 a 8,3 % žen s BRCA2 kombinaci obou nádorů. Nejčastěji nalezenou sekvenční variantou byla varianta c. 5266dupC v genu BRCA1. Ženám s potvrzenou patogenní mutací bylo nabídnuto prediktivní testování celé rodiny, ale pouze 20 rodin (9,4 %) tuto možnost využilo. Pozitivně testovaných příbuzných bylo téměř 70 %. Závěr: Ze studie vyplývá úvaha o příčinách a potřebách vyšetřování jak pacientek, tak žen s predispozicí k ca prsu. Veškeré poznatky získané během této studie byly využity v rámci rutinní praxe na oddělení Lékařské genetiky, Laboratoře AGEL a.s.

Abstract:

Introduction: Breast cancer (ca) is currently one of the most discussed and also the most common of malignant tumors in women in the Czech Republic. Up to 7,200 new cases are diagnosed in the Czech Republic every year. Incidence is constantly increasing. Risk factors that contribute to the development of breast cancer include hormonal, nutritional environmental and genetic factors. The genes that are most associated with breast cancer and ovarian cancer too are called the BRCA1 and BRCA2 genes. The lifetime cumulative risk of developing breast cancer in women with a confirmed mutation in the BRCA1/2 genes ranges from 55 to 85 %, according to Czech statistics. Aim: The aim of this study was to detect the presence of a pathogenic sequence variant in the BRCA1/2 genes or another gene. Last aim was a follow up on the result and connective on appropriate preventive care. Methods: With the development of laboratory diagnostics massive parallel sequencing (NGS) methods are now commonly used to detect mutations, which also allow the detection of other pathogenic sequence variants or variants of uncertain signifikance that have not been previously detected. The study used various methods of laboratory analysis in 2033 women, mostly from the Moravian-Silesian region. Results: There were 2033 Czech women included in the molecular genetic analysis and 212 pathogenic mutations were detected. 127 women were positive in the BRCA1 gene (60%), 72 women in the BRCA2 gene (34%) and 13 women in other candidate genes (CHEK2, PALB2, ERCC4) (6%). The results show that 74 % of women with a confirmed mutation in the BRCA1 and 83,3 % of women with BRCA2 gene already had breast cancer. 21,3 % of women with BRCA1 and 15,3 % of women with BRCA2 gene had ovarian cancer and 4,7 % of women with BRCA1 and 8,3 % of women with BRCA2 had a combination of both tumors. The most frequent sequence variant was c.5266dupC in the BRCA1 gene. For women with a confirmed pathogenic mutation were offered targeted mutation testing of their family. Only 20 families (9,4 %) used the possibility to be tested. Almost 70% of the relatives tested were positive. Conclusion: The study results suggest a reflection of the causes and needs for examination of patients and women predisposed to breast or ovarian cancer. All findings obtained during this study were used in routine practice at the Department of Medical Genetics, Laboratory of AGEL a.s.
