Abstract:

Nádorová imunoterapie využívá imunitní systém k boji proti nádorovým onemocněním. Hlavním předpokladem je stimulace imunitního systému pacienta k útoku proti maligním nádorovým buňkám odpovědným za nemoc. Toho je dosaženo buď imunizací pacienta nebo prostřednictvím podání terapeutických protilátek. Buněčná imunoterapie je dalším významným subjektem nádorové imunoterapie. Zahrnuje buňky imunitního systému - NK buňky, cytotoxické T lymfocyty, dendritické buňky apod., které jsou aktivovány buď in vivo uplatňováním některých cytokinů nebo jsou izolovány, obohaceny a přeneseny zpět pacientovi k boji proti nádorovému onemocnění. Dendritické buňky jsou profesionální antigen prezentující buňky, jsou vysoce specializované v zachycení, zpracování a prezentaci antigenu. Dendritické buňky kultivované a loadované antigenem indukují antigen specifickou imunitu. Od této formy imunoterapie se očekává identifikace mechanismů kontroly imunogenicity dendritických buněk a lepší pochopení buněčné dynamiky. I přes nedávný pokrok v léčbě leukemií u mnoha pacientů dochází k relapsu, proto je potřeba vyvíjet nové léčebné strategie. Identifikace antigenů asociovaných s leukémií a pokrok v pochopení základů imunologie nádorů přináší pokrok ve vakcinaci pomocí peptidů.

Abstract:

Cancer immunotherapy is the use of the immune system to reject cancer. The main premise is stimulating the patient's immune system to attack the malignant tumor cells that are responsible for the disease. This can be either through immunization of the patient or through the administration of therapeutic antibodies. Cell based immunotherapy is another major entity of cancer immunotherapy.This involves immune cells such as the Natural killer Cells, Cytotoxic T Lymphocytes, Dendritic Cells etc. which are either activated in vivo by administering certain cytokines or they are isolated, enriched and transfused to the patient to fight against cancer. Dendritic cells are professional antigen-presenting cells with, they are highly specialized in antigen capture, processing and presentation antigens. Dendritic cells generated in culture and loaded with an antigen efficiently induce antigen-specific imunity. From this form of immunotherapy is expected following the identification of the mechanisms controlling dendritic cell immunogenicity and a better understanding of the cell dynamics. Despite recent advances in the treatment of leukemia, many patients will relapse and novel therapeutic strategies are therefore needed. The identification of a number of leukemia-associated antigen (LAA) and advances in understanding the fundamentals of tumor immunology have fostered the progress in vaccination using peptides.
