Radiosensitivity of Liver

Abstract:

Cíl : Prokázat, že játra mají dostatečně vysokou toleranci k ozařování, aby bylo možno využít hyperfrakcionované radioterapie k léčbě inoperabilních jaterních malignit, především pak hepatocelulárního karcinomu. Metodika : Dvě případové kazuistiky pacientek s neoperovatelnou formou hepatocelulárního karcinomu, u kterých byla provedena vysokodávková radioterapie do poměrně malého objemu. Hlavním zdrojem informací při tvorbě kazuistik byly chorobopisy pacientek. Výsledky : Výsledky obou kazuistik jsou velice povzbudivé. Obě pacientky absolvovaly hyperfrakcionovanou radioterapii o LD = 65Gy ( v prvním případě ) a 75Gy ( v druhém případě ), celá léčba probíhala standardním způsobem bez výrazných potíží. Po ukončení radioterapie došlo u obou případů k regresi onemocnění a k podstatnému zlepšení celkového stavu. Závěr : V historii byla játra považována za extrémně radiosenzitivní orgán, ale v dnešní době je již zcela jasné, že tomu tak není. Moderní radiobiologie považuje játra za typicky paralelní orgán, kdy o stupni poškození při ozařování rozhoduje střední ozařovací dávka na celá játra. Parciální tolerance jater k radioterapii je velice vysoká. Za zcela bezpečnou dávku se v současnosti považuje 60Gy na 50 % a až 75Gy na 30 % jater. Z kazuistik vyplývá, že lze do jater úspěšně dodat poměrně vysokou dávku ionizujícího záření s velice uspokojivými výsledky, za předpokladu, že je ušetřena značná část normálních jater. Avšak hyperfrakcionovaná ozařovací dávka musí být podána do malé části jinak zdravých jater

Abstract:

AIM: The purpose of the study is to demonstrate that tolerance of the liver to ionizing radiation is higher than originally expected, so that we can use high dose radiotherapy for the treatment of unresectable hepatic malignancies, especially hepatocellular carcinoma. METHODICS: I used two case reports of patients (two women) with unresectable hepatocellular carcinoma. High dose radiotherapy was administered to a relatively small volume of liver in both patients. Clinical records of patients were used for the evaluation of acute toxicity, late toxicity and the outcome. RESULTS: Results of case studies are very encouraging in both women. The patients were given hyperfractionated radiotherapy to the total dose of 65 Gy in the first case and 75 Gy in the second case. The treatment was well tolerated without significant toxicity. After the completion of radiotherapy, a significant treatment response was recorded and the performance status of both patients had improved markedly. CONCLUSION: Historically, liver was thought to be a supersensitive organ to irradiation, but recent data have changed this paradigm. According to recent radiobiological reports, liver is considered to be a typical parallel organ so that mean radiation dose to the whole liver decides about the level of damage. Partial tolerance of the liver to radiation is very high. 60 Gy for 50 % and 75 Gy for 30 % of the liver are allowed to be safe dose. From case studies results, a conclusion can be drawn that we can administer a relatively high dose of ionizing radiation to a part liver with very encouraging results, but large part of normal liver must be protected.
