Inflammation and its Resolution in Atherosclerosis

Abstract:

Ateroskleróza je chronický zánětlivý proces postihující arteriální cévní stěnu, kdy konečný klinický výsledek určuje rovnováha prozánětlivých a zánětlivých mechanismů. Rozvoj aterosklerózy je iniciován infiltrací a modifikací lipoproteinů a jejich absorpcí pomocí makrofágů, s následnou tvorbou lipidových pěnových buněk, které iniciují tvorbu aterosklerotických lézí a nedostatečné eferocytotické odstranění apoptotických a pěnových buněk udržuje progresi lézí. Vadná eferocytóza udržuje progresi lézí a vede k akumulaci sekundárně nekrotických makrofágů a pěnových buněk a tvorbě pokročilých lézí, až po jejich destabilizaci a stádium trvajícího zánětu. Rezoluce zánětu má důležitou roli v ukončení zánětlivé odpovědi a je zprostředkována pomocí specializovaných pro-rezolučních lipidových mediátorů. Jedním z hlavních účinků mediátorů rezoluce zánětu je fenotypová přeměna prozánětlivých makrofágů na makrofágy, které potlačují zánět a podporují hojení.

Abstract:

Atherosclerosis is a chronic inflammatory process which affects the arterial vessel wall, when the balance of pro-inflammatory and inflammation-resolving mechanisms dictates the final clinical outcome. The development of atherosclerosis is initiated by infiltration and modification of lipoproteins and their absorption by macrophages, with ensuing formation of lipid-filled foam cells, initiate atherosclerotic lesion formation, and deficient efferocytotic removal of apoptotic cells and foam cells sustains lesion progression. Defective eferocytosis maintains the progression of the lesions and leads to the accumulation of secondary necrotic macrophages and foam cells and the formation of advanced lesions, only after their destabilization and the stage of persistent inflammation. One of the major effects of inflammation resolution mediators is phenotypic conversion of pro-inflammatory macrophages into macrophages that suppress inflammation and promote healing.
