Stability of urinary calculi

Abstract:

Na základě laboratorních experimentů byly studovány podmínky vzniku močových ka-menů a jejich stabilita v rozmezí pH typického pro lidskou moč. V rámci experimentální části byly připraveny 3 typy roztoků: (1) roztok fosforečnanu vápenatého, (2) roztok šťavelanu vápenatého a (3) kombinovaný roztok obou látek. Analýzou těchto roztoků byl získán soubor dat, který byl následně dále zpracován pomocí modelovacího progra-mu Geochemists Workbench®. Prostřednictvím stabilitních diagramů byly vymezeny podmínky, za kterých jsou jednotlivé typy močových kamenů stabilní. Výsledky ukázaly proměnlivost minerálů srážejících se v jednotlivých typech roztoků. Porovnáním vý-sledků se vzorkem lidské moči bylo zjištěno, že moč zdravého člověka může obsahovat i vyšší množství rozpuštěných fosforečnanových, šťavelanových, nebo vápenatých látek než je třeba ke tvorbě konkrementů a přitom nemusí dojít ke vzniku močových kamenů. Dále bylo prokázáno, že rozpustnost šťavelanů je v termodynamické databázi podceně-na. V případě fosforečnanu i šťavelanu vápenatého (po úpravě databáze) odpovídaly modely výsledkům laboratorních experimentů a je tedy možné je využít pro detailnější studium této problematiky.

Abstract:

Based on laboratory experiments, the conditions of urinary stones formation and their stability in the pH range typical for human urine were studied. Three types of solutions were prepared in the experimental stage: (1) solution of calcium phosphate, (2) solution of calcium oxalate and (3) combined solution of both substances. The analysis of these solutions provided the set of data, which was then processed using the program Geo-chemist's Workbench®. The stability conditions of individual types of urinary stones were defined by stability diagrams. Results showed the variability of minerals precipita-ting in individual types of solutions. The comparison of results with human urine sample showed that urine of healthy human can contain higher amount of dissolved phosphates, oxalates or calcium substances than amount necessary for concrements formation, but it may not lead to creation of urinary stones. Furthermore, it was shown that the solubility of calcium phosphate and calcium oxalate in thermodynamic database was underestimated. After editing the thermodynamic databases, the models of both substances corresponded to the results of laboratory experiments, which allows using them for more detailed study of this issue.
