Lyme disease

Abstract:

Lymeská borrelióza je chronické multisystémové infekční onemocnění s poměrně vysokou incidencí, přenášené klíšťaty. Původcem choroby je komplex spirochetálních bakterií Borrelia burgdorferi sensu lato. V České republice (ČR) je každým rokem hlášeno přibližně 4 000 případů. Přestože je lymeská borrelióza (LB) relativně mladá klinická jednotka, poznatky o ní jsou poměrně široké. Ačkoli již byla definována podstata onemocnění, patogeneze, klinické formy i způsoby léčby, stále probíhá intenzivní výzkum a názory na problematiku onemocnění se stále vyvíjí. LB je totiž onemocnění s obtížnou diagnostikou a díky široké škále projevů, které ji provází, často působí lékařům potíže. Právě pro tuto unikátní různorodost symptomů bývá i efekt léčby problematický, mnohdy se zbytečně nasazenou antibiotickou terapií. Vzhledem k tomu, že dosud není k dispozici spolehlivá očkovací látka, je jedinou možností, jak omezit riziko nákazy, pečlivá ochrana před klíšťaty. Práce se snaží shrnout současné poznatky o historii, etiologii, epidemiologii, patogenezi, klinice, diagnostice a terapii lymeské borreliózy.

Abstract:

Lyme disease is a chronic multisystem infective disease, with relatively high incidence, transmitted by ticks. Causer of the disease is a complex of spirochete bacteria Borrelia burgdorferi sensu lato. In the Czech Republic there is about 4 000 causes every year reported. Although Lyme disease is relatively young clinical unit, the findings about it are reasonably wide. Though the essence of this disease was defined, pathogenesis, clinic forms and way of treatment, there is still an intense research ongoing and the opinions about the problematics of disease are still evolving. Lyme disease is uneasily diagnosed. Thanks to wide scale of symptoms which causes difficulties to doctors. Because of this unique variety of symptoms the effect of treatment is problematic, often unnecessarily cured with antibiotic therapy. Due to the fact, that there is no reliable vaccine available, the only option how to restrict the risk of contagion is a careful tick protection. This thesis is trying to summarize findings about history, aetiology, epidemiology, pathogenesis, clinic, diagnostics and therapy of Lyme disease.
