Nursing Process in the Care of Patient after Total Laryngectomy

Abstract:

Bakalářská práce se věnuje problematice komplexní ošetřovatelské péče u pacienta po totální laryngektomii, popisuje převážně pooperační období. Cílem bakalářské práce je vypracování ošetřovatelského procesu u konkrétního pacienta, který podstoupil rozšířený operační výkon, totální laryngektomii v souvislosti s onkologickým onemocněním hlavy a krku. Součástí operace bylo odstranění štítné žlázy, spádových lymfatických uzlin a vyvedení trvalé tracheostomie. Plán ošetřovatelské péče byl zaměřen převážně na pooperační péči. Posouzení, diagnostika, intervence, realizace a hodnocení ošetřovatelské péče, vycházející z anamnestických údajů pacienta a stanovených ošetřovatelských diagnóz dle NANDA-I Taxonomie II. Důležitým krokem k výběru ošetřovatelských diagnóz bylo právě posouzení pacienta. Bakalářská práce se dělí na část teoretickou a praktickou. Teoretická část má dvě kapitoly, praktická část kapitolu jednu. Praktická část předkládá ošetřovatelský plán, který byl sestaven sběrem dat z anamnézy pacienta, ze zdravotnické dokumentace, strukturovaným rozhovorem, subjektivním (pacient) i objektivním (všeobecná sestra) pozorováním a následně doplněnými údaji ošetřovatelského týmu daného oddělení. V plánu ošetřovatelské péče bylo stanoveno 11 ošetřovatelských diagnóz zaměřených na problém (5), podporu zdraví (2) a rizikové diagnózy (4). Z těchto ošetřovatelských diagnóz bylo vybráno šest stěžejních: 1) narušené polykání - pacientovi byl zaveden PEG, 2) neefektivní vzorec dýchání - pacientovi byla vytvořena trvalá TCHS, 3) narušená verbální komunikace, 4) riziko infekce chirurgického místa, 5) narušena integrita ústní sliznice, 6) akutní bolest, které byly popsány v praktické části bakalářské práce. Diagnózy byly na konci vyhodnoceny, z toho pět ošetřovatelských diagnóz bylo splněno a ukončeno, jedna ošetřovatelská diagnóza nebyla ukončena, ale cíl splněn. Odůvodnění neukončené diagnózy je popsáno v hodnocení.

Abstract:

Bachelor's thesis deals with the issue of comprehensive nursing care for patients after total laryngectomy, with a description mainly of the postoperative period. The aim of the bachelor's thesis is to develop a process for care of a specific patient who has undergone an extended total laryngectomy, due to head and neck cancer. The operation included removal of the thyroid gland, descending lymph nodes and removal of the permanent tracheostomy. The nursing care plan focuses mainly on postoperative care. Assessment, diagnosis, intervention, implementation and evaluation of nursing care, are based on patient's anamnestic data and established nursing diagnosis according to NANDA-I Taxonomy II. An important step in choosing a nursing diagnosis was the assessment of the patient's condition. Bachelor's thesis is divided into theoretical and practical parts. The theoretical part consists of two chapters, the practical part consists of one chapter. The practical part presents a nursing care plan, which was compiled by collecting data from the patient's medical history, records, structured interviews, subjective (patient) and objective (general nurse) observations and supplemented by data from the nursing team of the department. The nursing care plan identified 11 nursing diagnoses focusing on problem (5), health promotion (2) and risk diagnosis (4). Six of these nursing diagnoses were selected: 1) impaired swallowing - the patient was introduced PEG, 2) ineffective breathing pattern - the patient developed permanent TCHS, 3) impaired verbal communication, 4) risk of infection of surgical site, 5) impaired oral mucosa, 6) acute pain, which were described in the practical part of bachelor's thesis. Final evaluation of the diagnosis was made when five nursing diagnoses were fulfilled and completed, one nursing diagnosis was not completed, but the goal was achieved. The rationale for the incomplete diagnosis is described in the evaluation.
