Physiotherapy for arthrosis of the joints of the lower limbs

Abstract:

Úvod: Za téma své bakalářské práce jsem si vybrala fyzioterapii při artróze kloubů dolních končetin. Cíl práce: Cílem práce je přispět k problematice artrózy kloubů dolních končetin, výběrem vhodných konceptů a metodik fyzioterapie u pacientky s gonartrózou III.st u obou dolních končetin. Metodika: Hlavní část mé bakalářské práce se zabývá kazuistikou pacientky s gonartrózou III.st. u obou dolních končetin. S pacientkou jsem spolupracovala 3 měsíce, vždy 1x týdně, tedy absolvovala 12 návštěv. Před zahájením fyzioterapeutické intervence jsem provedla vstupní kineziologický rozbor (vyšetřila jsem aspekcí, palpací, goniometrii dolních končetin, antopometrii dolních končetin, svalový test dle Jandy, zkrácené struktury, stoj a pohybový stereootyp chůze), na jehož základě jsem vypracovala krátkodobý fyzioterapeutický plán. V průběhu fyzioterapeutických intervencí jsem provedla kontrolní vyšetření (goniometrii a antropometrii DKK, svalový test dle Jandy, vyš. pohybového stereotypu do abdukce a extenze v kyč.kloubu), po šesti týdnech. Náplní cvičebních jednotek bylo využití fyzioterapeutických konceptů a metodik (míčkování dle Jebavé, aktivní cvičení, cvičení dle svalového testu dle Jandy, postizometrická relaxace, postfacilitační strečink, proprioceptivní neuromuskulární facilitace, senzomotorická stimulace, cvičení s Thera-bandem dle Brügger konceptu, cvičení v uzavřeném a otevřeném kinematickém řetězci, využití různých pomůcek a jiné), k ošetření měkkých tkání, k protažení zkrácených struktur, nácviku správného stereotypu chůze a stability, ke zlepšení svalové síly a zvýšení rozsahu pohybu v kolenním kloubu. Výsledky: Dle výstupního kineziologického rozboru došlo oproti vstupnímu vyšetření ke snížení otoku u levého kolene, nad kolenem a přes tuberositas tibie o 1 cm, přes kolenní kloub o 2 cm. Rozsah pohybu do flexe na PDK se zvýšil aktivně i pasivně o 5st a u LDK aktivně i pasivně o 10st. Pohyblivost pately byla obnovena kraniokaudálním směrem u PDK a mediolaterálním i kraniokaudálním směrem u LDK. Blokáda hlavičky fibuly na PDK již není přítomna. Svalová síla se zvýšila u m. quadriceps femoris o 1st u obou dolních končetin a u m. iliopsoas ze stupně 4 na 4+ u obou dolních končetin. Závěry: Cíl a úkoly mé práce jsem během terapií s pacientkou splnila.

Abstract:

Introduction: I chose physsiotherapy for lower limb arthrosis as the topic of my bachelors thesis. Th Aim: The aim of my bachelor thesis is to contribute to the issue of arthrosis of the joints of the lower limbs by selecting appropriate concepts and methods of physiotherapy in a patient with gonarthrosis of III. degrees. Methodology: The main part of my bachelors thesis deals with the case report of a patient with a gonarthrosis of III. degrees in both lower limbs. I worked with the patient for 3 months, she completed 12 visits. Before starting the physiotherapeutic intervention, I performed an initial kinesiological analysis (I examined the aspect, palpation, goniometry ant anthropometry of the lower limbs, muscle test according to Janda, shortened structures and movement stereotype of walking), on this basis I developer a short time physiotherapy plan. During physiotherapeutic intervention I prerformed follow - up examinations after 6 weeks. The content of the exercise units was the application of physiotherapeutic concepts and methodologies (method according to Jebavá, active exercise, Janda muscle test exercise, postisometric relaxation, postfacilitation stretching, proprioceptive neuromuscular facilitation, sensorimotor stimulation, Brügger concept, exercises in closed and open kinematic string, use of various tools and others), to.treat soft tissues, to stretch shortened structures, to practice the right stereotype of walking and stability, to improve muscle strenght and increase the range of motion in the knee joint. Results: According to the final kinesiological analysis, it occurred, compared to the initial analysis to reduce swelling in the left lower limb, above the knee a nd through the tuberositas of the tibia by 1 cm, over the knee joint by 2 cm. The range of motion in flexion increased active and passive by 5 degrees on the right lower limb, on the left lower limb aktive und passive by 10 degrees. I restored patellar mobility in the craniocaudal direction in the right lower limb and in the mediolateral and craniocaudal directions in the left lower limb. Block of the caput fibulae is no longer present on the right lower limb. The quadriceps femoral muscle has 1 degree greater strength on both lower limbs and in the iliopsoas muscle, musclestrengh ranges from degree 4 to degree 4+ on both lower limbs. Conclusions: I fulfilled the goals and tasks of my work during the thetrapy with the patient.
