Women's obstetric injuries during vaginal delivery and their prevention

Abstract:

Bakalářská práce se zabývá problematikou poranění ženy po vaginálním porodu, zejména jeho prevencí. Teoretická část sumarizuje nejdůležitějších informace, týkající se této problematiky. Uvádí druhy poranění, které po vaginálním porodu vznikají nejčastěji a rizikové faktory pro jejich vznik. Zaměřuje se na možnosti prevence jak před porodem, tak v jeho průběhu a přináší základní informace o hojení ran a péče o ně. Praktická část je zaměřena na zhodnocení efektu prevence pomocí vaginálních dilatačních balónků. Hlavním cílem bylo zjistit, zda jsou přínosnou prevencí pro vznik porodních poranění a mají efekt na zkrácení druhé doby porodní. Vedlejšími cíli bylo prozkoumat, zda jsou ženy o této metodě prevence dostatečně edukovány, zmapování, jakým způsobem cvičí s vaginálním dilatačním trenažérem a jestli pomůcku využívají i pro rehabilitaci po porodu. Dále také zjištění subjektivního názoru porodních asistentek na vaginální dilatační trenažery. Výsledným zjištěním je, že vaginální dilatační trenažér má efekt na prevenci poranění a zkrácení druhé doby porodní. Ženy však nejsou dostatečně edukovány odborníky, zejména porodními asistentkami, což má za následek, že ve většině případů jej nevyužívají pro poporodní rekonvalescenci. Porodní asistentky nemají ucelený názor na účinky pomůcky. Avšak ve více než polovině případů jej doporučují těhotným ženám využívat.

Abstract:

This bachelor´s thesis deals with the issue of woman´s injuries after a vaginal delivery, especially with their prevention. The theoretical part summarizes the most relevant information on this problematic. It lists the types of injuries which occur most frequently after the vaginal delivery and the risk factors for their occurrence. It focuses on the prevention options both before and during childbirth and it provides the basic information on wound healing and care. The practical part aims at the evaluation of the effect of prevention throughout vaginal dilatation balloons. The main objective was to find out if they are a beneficial prevention for birth injuries and have the effect of shortening the second stage of labor. The secondary objectives were to find out whether women are sufficiently educated about this prevention method, mapping how they train with the vaginal dilatation trainer, and whether they used the tool for their rehabilitation after the childbirth. Furthermore, also finding a subjective opinion of midwives on vaginal dilatation trainer. The outcome is that the vaginal dilatation trainer has an effect on preventing injury and shortening the second stage of labor. However, women are not adequately educated by professionals, especially by midwives, result of which is that women do not use it for postnatal reconvalescence in most cases. The midwives do not have a comprehensive opinion on the effects of this tool. However, they recommend its usage to pregnant women in more than half of the cases.
