Who's to take care of births?: The public controversy over the professionalization of Czech midwifery

Abstract:

Diplomová práce je případovou studií profesionalizace porodní asistence v ČR, kterou zkoumá v kontextu veřejné kontroverze kolem změn v současném českém porodnictví. Úvodní kapitoly práce se zabývají současností a historií porodní asistence, otázkami „přirozeného“ a lékařsky vedeného porodu a tématem profesionalizace porodní asistence. Analýza je věnována rozboru dvou debat (veřejné Masarykovy debaty s názvem „Nemocnice je jediné bezpečné místo pro porod“ a semináře pořádaného Výborem pro zdravotnictví Poslanecké sněmovny Parlamentu ČR s názvem „Domácí porody – krok kupředu či zpět“) a nálezu Ústavního soudu v případě soudního sporu s nejznámější porodní asistentkou Ivanou Königsmarkovou. V této části práce bude zmapována a sociologicky rozebírána argumentace stran angažovaných ve veřejné kontroverzi ohledně toho, kdo má vést porod a jak by porod v současné době měl vypadat. Na pozadí tohoto sporu se totiž vyjednávají klíčové otázky porodní asistence jako samostatné profese – otázky odbornosti, vzdělávání a spolupráce s dalšími profesemi.

Abstract:

This thesis is a case study of profesionalization of birth midwifery in the Czech Republic. It is researched in the context of a public controversy, surrounding changes of the contemporary czech obstetrics. The initial chapters of this thesis inquire into the current state and the history of the midwifery, the questions of „natural“ and physician-led births and the theme of the birth assistance profesionalization. The analysis sets out to examine two of the debates (a public Masaryk's debate titled „The Hospital is the Only Safe Place to Give Birth in“ and a seminar, organized by the Committee for Health of the Chamber of Deputies of the Czech Republic, titled „Home Births – a Step Forward or Backward“) and a ruling of constitutional court in the case of litigation with the best-known midwife Ivana Königsmarková. This part of the work will survey and sociologically analyze the argumentation of both the sides, engaged in the public controversy about the questions of who should lead the births and what should the present-day births look like. Significantly, the quarrel reflects ongoing negotiation of the key questions of midwifery as an autonomous profession – the questions of expertness, education and training, and cooperation with the other professions.
