Health Literacy in selected population groups in the Moravian-Silesian Region

Abstract:

Úvod: Zdravotní gramotnost je významným ukazatelem v oblasti veřejného zdraví, který hodnotí úroveň schopnosti jedince získávat, porozumět, vyhodnocovat a používat informace, které se týkají jeho zdraví. Jedinec s dobrou úrovní zdravotnických znalostí by měl být schopen využívat informace způsobem, který je prospěšný pro jeho zdraví. Dle průzkumu z roku 2014 označilo více než 40 % respondentů v České republice zdravotní gramotnost za problematickou nebo nedostatečnou. Cílem praktické části je vyhodnocení zdravotní gramotnosti u souboru z obecné populace a osob sociálně vyloučených, srovnání jednotlivých skupin podle faktorů a srovnání zdravotní gramotnosti těchto skupin s celkovou zdravotní gramotností v ČR. Metodika: Při šetření byl použit zkrácený dotazník HLS-EU-Q16. Sběr dotazníků u osob sociálně vyloučených proběhl v Opavské věznici, u obecné populace byl použit tzv. sběr sněhové koule. Zdravotní gramotnost byla vypočtena součtem bodů přiřazeným k jednotlivým odpovědím v dotazníku. Na základě počtu bodů byly definovány 3 stupně zdravotní gramotnosti. Pro statistické vyhodnocení byly použity základní statistické charakteristiky. Pro srovnání údajů byl použit Fisherův exaktní test, chí-kvadrát test a Mann- Whitney test. Dále byl použit model logistické regrese, byly vypočteny plně adjustované OR. Testy byly hodnoceny na hladině významnosti 5 %. Pro zpracování dat byl použit program Stata verze 17. Výsledky: Z výsledků vlastního výzkumu vyplývá, že u osob sociálně vyloučených (33,9 %) a obecné populace (38,9 %) v rámci zdravotní gramotnosti nevyšel statisticky významný rozdíl. Při srovnání zdravotní gramotnosti dle sledovaných faktorů byl zjištěn statisticky významný vztah pouze u zdravotního stavu. Jedinci uváděli problematickou a neadekvátní zdravotní gramotnost především u průměrného a špatného zdravotního stavu. Osoby sociálně vyloučené a obecná populace nejčastěji uváděly dostatečnou zdravotní gramotnost. Závěr: Srovnání vlastního výzkumu v rámci jednotlivých šetření v ČR ukázalo podobný statisticky významný vztah mezi zdravotním stavem a zdravotní gramotností. Jedinci s vyšší úrovní zdravotní gramotnosti vykazovali lepší zdravotní stav. Co se týče úrovně zdravotní gramotnosti došlo ke zlepšení oproti výzkumu v Jihočeském kraji a šetření v ČR roku 2014.

Abstract:

Introduction: Health literacy is an important public health indicator that assesses the level of an individual's ability to acquire, understand, evaluate and use health-related information. An individual with a good level of health literacy should be able to use information in a way that is beneficial to their health. According to a 2014 survey, more than 40 % of respondents in the Czech Republic identified health literacy as problematic or insufficient. The aim of the practical part is to evaluate health literacy in a sample of the general population and socially excluded persons, to compare each group according to factors and to compare the health literacy of these groups with the overall health literacy in the Czech Republic. Methodology: Health literacy was calculated by summing the score assigned to each answer in the questionnaire. Based on the number of points, 3 levels of health literacy were defined. Basic statistical characteristics were used for statistical evaluation. Fisher's exact test, chi- square test and Mann-Whitney test were used to compare the data. In addition, a logistic regression model was used and crude odds ratios (OR) and fully adjusted OR were calculated. The tests were evaluated at 5 % significance level. Stata version 17 was used for data processing. Results: The results of our own research show that there was no statistically significant difference in health literacy between the socially excluded (33.9%) and the general population (38.9%). When comparing health literacy by the factors studied, a statistically significant relationship was found only for health status. Individuals reported problematic and inadequate health literacy especially for average and poor health status. Socially excluded individuals and the general population were most likely to report adequate health literacy. Conclusion: Individuals with higher levels of health literacy reported better health status. In terms of health literacy levels, there was an improvement compared to the South Bohemia survey and the 2014 Czech survey.
