Nursing Process in Care of Patients with Morbus Crohn

Abstract:

Tato bakalářská práce se zabývá problematikou Crohnovy choroby se zaměřením na ošetřovatelskou péči u pacientů s touto diagnózou. Práce je členěna na dvě části - teoretickou a praktickou. Cílem teoretické části bylo přiblížení informací o této nemoci. Tato část tedy popisuje základní charakteristiku Crohnovy nemoci, potenciální příčiny vzniku, její projevy, komplikace i diagnostické a léčebné metody. Jsou zde také uvedena specifika ošetřovatelské péče u pacienta s tímto onemocněním. Hlavním cílem části praktické bylo zejména sestavení ošetřovatelského procesu u konkrétního pacienta s Crohnovou chorobou na základě ošetřovatelských diagnóz, dle NANDA-I, s ohledem na individuální potřeby. Tato část je tedy zaměřena na zpracování ošetřovatelského procesu a je zakončena diskuzí. Vybraným pacientem byl muž F.F. ve věku 28 let s již dříve diagnostikovanou Crohnovou chorobou, který byl hospitalizovaný z důvodu relapsu onemocnění a vzniklých komplikací na chirurgickém oddělení. Od přijetí bylo u pacienta stanoveno 10 stěžejních ošetřovatelských diagnóz, přičemž byl u každé této diagnózy stanoven ošetřovatelský cíl a popsány vhodné intervence, následná realizace a hodnocení ošetřovatelské péče. Informace, na základě kterých byly stanoveny ošetřovatelské diagnózy, byly získány metodou rozhovoru s pacientem i se zdravotnickým personálem na daném oddělení, vlastním pozorováním pacienta v průběhu hospitalizace a také díky studiu ošetřovatelské i lékařské zdravotnické dokumentace. Celkově byl pacient hospitalizován po dobu 14 dní a po této době byl ve stabilizovaném stavu propuštěn do péče praktického lékaře s následnými kontrolami v chirurgické a gastroenterologické ambulanci.

Abstract:

This bachelor's thesis deals with the issue of Crohn's disease, focusing on the nursing care for patients with this diagnosis. The work is divided into two parts - theoretical and practical. The aim of the theoretical part was to provide information about this disease therefore it describes the basic characteristics of Crohn's disease, its potential causes, its symptoms, complications as well as diagnostic and treatment methods. The specifics of nursing care for a patient with this disease can be also found there. The main goal of the practical part was, in particular, to adjust a nursing process for a specific patient with Crohn's disease, based on nursing diagnoses, according to NANDA-I, with regard to individual needs. This part is therefore focused on the processing of the nursing process and concluded with a discussion. The chosen patient was a man called F.F. at the age of 28, with previously diagnosed Crohn's disease, who was hospitalized due to the relapse of the disease and complications arising in the surgical ward. Since patient's reception, ten core nursing diagnoses were established for him, while for each of these diagnoses, a nursing goal was stated and appropriate interventions described, followed by implementation and evaluation of nursing care. The information, leading to the nursing diagnoses, was obtained by interviewing the patient and the medical staff in the given department, by observing the patient during hospitalization, and also by studying the nursing and medical records. In total, the patient was hospitalized for 14 days, afterwards he was allowed home in a stable condition to be taken care of a general practitioner with follow-up examinations at the surgical and gastroenterology ambulance.
