Balloon dilation of the Eustachian tube for chronic obstructive Eustachian tube dysfunction

Abstract:

Úvod: Chronická dysfunkce Eustachovy tuby (ET) u dospělých pacientů je častou patologií, se kterou je spojen rozvoj mnoha onemocnění. Terapeutické možnosti jsou velmi omezené, povětšinou symptomatické. V roce 2010 byla v zahraničí představena balónková dilatace Eustachovy tuby (BET), jejímž principem je dilatace tuby vedoucí k návratu přirozené ventilační a drenážní cesty středouší. Jedná se o kauzální léčebnou modalitu, která je ale stále nová a není známa její úspěšnost u jednotlivých skupin pacientů s chronickou dysfunkcí ET. Není také známo, zda je výhodou u chronické sekretorické otitidy (OMS) doplnit zákrok na bubínku. Cílem předkládané studie bylo zjistit efekt BET u jednotlivých skupin pacientů s chronickou dysfunkcí ET a srovnat její efekt u pacientů s OMS se současným provedením tympanocentézy a bez ní. Metody: Do studie byly zařazeny dvě skupiny dospělých pacientů. Do první byli zařazeni pacienti s OMS, kteří alespoň jedenkrát podstoupili tympanostomii s návratem obtíží. Do druhé byli zařazeni pacienti s příznaky chronické dysfunkce ET při jinak fyziologickém nálezu. Všichni pacienti podstoupili BET. První skupina byla dále náhodně rozdělena na dvě podskupiny, u jedné podskupiny byla BET doplněna o tympanocentézu. Sledování proběhlo pomocí dotazníku, schopnosti provést Valsalvův a Toynbeeho manévr, tympanometrie a rozdílu ve vzdušně-kostním vedení při audiometrii. Výsledky: Úspěch BET u OMS (29 uší) byl sledován po 2, 6 a 12 měsících dle tympanometrie v 55 %, 48 % a 48 %, dle manévrů v 55 %, 41 % a 41 %, dle audiometrie v 69 %, 62 % a 59 %, dle dotazníku v 76 %, 72 % a 69 %. Došlo k signifikantnímu zlepšení nedoslýchavosti i dotazníkového skóre. Vše bylo nezávislé na provedení tympanocentézy. Úspěch BET ve druhé skupině (11 uší) byl v 75 %, 92 % a 75 %, rozdíl proti OMS nebyl signifikantní. Došlo k signifikantně výraznějšímu zlepšení dotazníkového skóre. Závěr: U obou skupin je možné očekávat dobrý efekt BET, který se jeví závislý na hodnoceném parametru a délce sledování. Výraznější efekt lze očekávat u prostých symptomů dysfunkce než u OMS. U OMS nelze rutinně doporučit současné provedení tympanocentézy.

Abstract:

Background: Chronic Eustachian tube (ET) dysfunction is a common pathology in adults, and an etiopathogenetic factor of many diseases of the middle ear. Yet, minimum therapeutic options address the cause of ET dysfunction. Recently, a novel causal therapeutic method called balloon dilation of the Eustachian tube (BET) has been developed. It is based on the dilation of the ET that leads to the restoration of proper middle ear ventilation and drainage. However, data are lacking on its effect in specific patient groups. There is also no clear conclusion on whether BET should be performed in combination with a procedure on the tympanic membrane in the case of chronic otitis media with effusion (COME). The aim of our study was to determine if there is a beneficial effect of BET combined with tympanocentesis compared to BET only in adults with COME, and provide specific data on the effect of BET in adults with different types of chronic ET dysfunction. Methods: Adults suffering from COME lasting more than six months after tympanostomy tube exclusion (group 1) and adults with symptoms of chronic ET dysfunction only and physiological clinical finding (group 2) were included in our study. All patients underwent BET. In addition, patients with COME were randomized into BET only and BET with tympanocentesis subgroups. Main outcome measures were tympanometry, Valsalva or Toynbee manoeuvre, questionnaire score, and pure-tone audiometry. Results: A total of 29 ears with COME were included in group 1. No significant difference in the effect of treatment was found between the subgroups. When patients were evaluated as one group, after 2, 6, and 12 months, the therapy was found to be successful in tympanometry in 55%, 48%, and 48%; in the ability to perform manoeuvres in 55%, 41%, and 41%; in audiometry in 69%, 62%, and 59%; and in questionnaire score in 76%, 72%, and 69% of cases, respectively. Questionnaire score and hearing loss were significantly reduced. The therapy was successful in 75%, 92%, and 75% of 11 ears in group 2. The difference in treatment success was insignificant, the reduction of questionnaire score was significantly better in group 2 compared to group 1. Conclusions: Generally, a good effect of BET can be expected. It appears to be dependent on the parameter evaluated and the length of follow-up. A more significant effect can be expected in the presence of symptoms of chronic ET dysfunction only. Concurrent tympanocentesis should not be routinely recommended in case of COME.
