Physiotherapy for total plastic surgery

Abstract:

Úvod: Práce se zabývá problematikou spojenou s náhradou kyčelního kloubu. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta s totální aloplastikou kyčelního kloubu. Metodika: Hlavní část této bakalářské práce popisuje kazuistiku pacientky po totální aloplastice pravého kyčelního kloubu. Spolupráce s pacientkou trvala po dobu její hospitalizace na rehabilitační klinice Malvazinky v Praze, což byly 3 týdny, během které proběhlo 12 terapií po 30 minutách. První fyzioterapeutická intervence začala týden po operaci, kdy byla nejprve odebrána anamnéza a byl proveden vstupní kineziologický rozbor (vyšetření aspekcí, palpací, antropometrie, goniometrie, statické a dynamické vyšetření kyčelního kloubu, pohybový stereotyp), na jehož základě byl stanoven krátkodobý fyzioterapeutický plán. Náplní fyzioterapeutických intervencí bylo využití fyzioterapeutických konceptů a metod (polohování, cévní gymnastika, respirační fyzioterapie, kondiční cvičení, pasivní pohyby, aktivní pohyby proti odporu, cvičení s pomůckami, mobilizace, techniky měkkých tkání, postizometrická relaxace, rytmická stabilizace, postfacilitační strečink, dynamická neuromuskulární stabilizace, proprioceptivní neuromuskulární facilitace, Brügger koncept - cvičení s therabandem). V závěru bylo provedeno výstupní kineziologické vyšetření, srovnání sledovaných parametrů, a na základě výsledků byl vytvořen dlouhodobý fyzioterapeutický plán. Výsledky: Pacientce se zlepšila svalová síla, zvýšil se rozsah pohybů včetně jejich způsobu provedení (nyní prováděno ve správném pohybovém stereotypu) a chůze je nyní zvládána samostatně s využitím 2 FH. Závěr: Díky zvolené metodice se podařilo splnit předem vytyčené cíle.

Abstract:

Introduction: The work deals with the issues associated with hip joint replacement. Objective: The aim of this bachelor's thesis is to contribute to the solution of physiotherapy issues in a patient with total hip arthroplasty. Methodology: The main part of this bachelor's thesis describes the case study of a patient after total hip arthroplasty of the right hip joint. The collaboration with the patient lasted for the duration of her hospitalization at the Malvazinky rehabilitation clinic in Prague, which was 3 weeks, during which 12 therapies of 30 minutes each took place. The first physiotherapeutic intervention started a week after the surgery, where the anamnesis was taken, and an initial kinesiological analysis was performed (examination of aspects, palpation, anthropometry, goniometry, static and dynamic examination of the hip joint, movement stereotype), based on which a short-term physiotherapeutic plan was established. The content of physiotherapeutic interventions was the use of physiotherapeutic concepts and methods (positioning, vascular gymnastics, respiratory physiotherapy, conditioning exercises, passive movements, active movements against resistance, exercises with aids, mobilization, soft tissue techniques, post-isometric relaxation, rhythmic stabilization, post-facilitation stretching, dynamic neuromuscular stabilization, proprioceptive neuromuscular facilitation, Brügger concept - exercises with theraband). In the end, an output kinesiological examination was performed, comparing the observed parameters, and a long-term physiotherapeutic plan was created based on the results. Results: The patient's muscle strength improved, the range of movements has increased, including the manner in which they are performed (now performed in the correct movement stereotype), and walking is now managed independently using 2 french sticks. Conclusion: Thanks to the chosen methodology, it was possible to meet the previously set goals.
