Use of a screening questionnaire of hearing handicap in primary care

Abstract:

Jedním z nejdůležitějším lidských smyslů je sluch, který poskytuje asi 60 % informací z okolního světa. Neporušená funkce sluchu je důležitá pro spojení a interakci jedince s okolním prostředím a má velký význam v mezilidské komunikaci. Nedostatek přijímaných informací poškozeným sluchem vyvolává psychické problémy a snižuje kvalitu života. Nedoslýchavost patří k nejčastějším onemocněním populace. Z hlediska závažnosti je řazena na 2. místo za postižením mentálním. Dle WHO je nejčastější a nejzávažnější smyslovou vadou. Cílem práce je porovnat subjektivní vnímání sluchové poruchy pomocí zkrácené verze dotazníku Hearing Handicap Inventory (HHIA-S) s naměřeným prahem sluchu pomocí tónové audiometrie a zjistit, zda je možné tento dotazník využít pro screening nedoslýchavosti u dospělých v ošetřovatelské praxi. Dotazník subjektivního vnímání sluchu se skládal z 10 uzavřených otázek. Sluchový hendikep byl vyhodnocen u 20 009 respondentů. Bylo vyšetřeno 10768 žen a 9241 mužů, kteří byli seznámeni s výzkumem a požádání o vyplnění dotazníku. Bylo provedeno otoskopické, následně audiometrické vyšetření sluchu včetně kostního vedení a tympanometrické vyšetření. Dotazník HHIA-S se hodnotil v závislosti na tíži nedoslýchavostí. Data byly statisticky zpracována. Shoda výsledků byla podle průměrných ztrát naměřených tónovou audiometrií a výsledného skóre sluchového hendikepu dle HHIA-S. Nejlepší shoda 92,2% byla dosažena v kategorii normální sluch, nejmenší shoda 49.9% byla dosažena u lehké a středně těžké doslýchavostí. Byla zjištěna poměrně vysoká senzitivita a specificita dotazníku. Senzitivita ve variantě ztrát sluchu od 26 dB byla 72,5% a specificita 92,2% v druhé variantě od 41 dB byla senzitivita 92,7% a specificita 80,4%. Včasné odhalení nedoslýchavosti a správná intervence jsou zásadní pro minimalizaci dopadu ztráty sluchu u nedoslýchavých. Pro vysokou senzitivitu a specificitu může dotazník HHIA-S sloužit jako nástroj pro screeningové vyšetření sluchu v ošetřovatelské péči i v ambulancích praktických lékařů.

Abstract:

One of the most important of human senses is hearing, which provides about 60% of the information from the outside world. Unimpaired function of hearing is important for relations and interaction of an individual with the surrounding environment and has great significance in interpersonal communication. Lack of information due impaired hearing causes psychological problems and reduces quality of life. Hearing loss is one of the most frequent diseases of the population. In terms of severity it is ranked in second place next to mental handicaps. According to WHO the hearing loss is the most common and most severe sensory defect. The aim of the study is to compare the subjective perception of hearing impairment using a shortened version of the questionnaire Hearing Handicap Inventory (HHIA-S) with the measured hearing threshold using tone audiometry and determine whether it is possible to use this questionnaire for screening of hearing loss in adults in nursing practice. Questionnaire of subjective perception of hearing consisted of 10 closed questions. Auditory disability was assessed at 20 009 respondents. The authors examined 10,768 women and 9241 men who were acquainted with the research and asked to complete a questionnaire. Otoscopy examination was carried out, followed by audiometric hearing screening including bone conduction and tympanometric examination. HHIA-S questionnaire was evaluated depending on the severity of hearing loss. Data were statistically processed. Congruence of results was according to average losses measured using tone audiometry and the final score of the hearing handicap according HHIA-S. Best congruence 92.2% was achieved in the category of normal hearing, the smallest congruence 49.9% was achieved in light and medium hearing loss. Relatively high sensitivity and specificity of the questionnaire was found. Sensitivity in variant of hearing loss from 26 dB was 72.5% and specificity of 92.2% in the second variant from 41 dB the sensitivity was 92.7% and specificity of 80.4%. Early detection of hearing loss and proper intervention are essential to minimize the impact of hearing loss. For the high sensitivity and specificity the HHIA-S questionnaire can be used as a tool for screening examination of hearing in nursing care and outpatient practitioners.
