Anatomical Brain Connectivity and Movement Sequencing in Schizophrenia

Abstract:

U pacientů se schizofrenií často nalézáme drobné neurologické odchylky, které označujeme jako měkké neurologické příznaky. Mezi nejčastější z nich patří porucha sekvencování pohybů. Tato práce se zaměřuje na studium neurobiologických podkladů poruchy sekvencování pohybů u pacientů se schizofrenií a testuje teorii kognitivní dysmetrie, tedy abnormitu cerebelární konektivity u schizofrenie. Studie se zúčastnilo 24 pacientů se schizofrenií a 23 zdravých dobrovolníků párovaných dle věku, pohlaví a preference ruky. Pacienti byli rozděleni dle NSS-SQ skóre na podskupinu s poruchou sekvencování komplexních motorických vzorců SQabn (n=7) a podskupinu bez těchto poruch SQnorm (n=17). Pomocí DTI traktografie byla rekonstruována vlákna pedunculus cerebellaris superior (SCP) a tractus corticospinalis (CST) a byly porovnány jednotlivé DTI parametry u zdravých dobrovolníků, pacientů SQabn a pacientů SQnorm. U podskupiny pacientů se schizofrenií bez poruch sekvencování pohybů (SQnorm) jsme nalezli narušení integrity kortikospinálního traktu vpravo. U podskupiny pacientů s poruchou sekvencování (SQabn) byla zjištěna abnormita pedunculus cerebellaris superior vlevo. Dále byla nalezena korelace mezi DTI parametry v pedunculus cerebellaris superior a závažností poruch sekvencování pohybů. Pomocí DTI traktografie lze odlišit podskupinu pacientů se schizofrenií s poruchami sekvencování pohybů a bez těchto poruch dle abnormalit ve specifických nervových drahách. Nález narušené integrity pedunculus cerebellaris superior u pacientů SQabn svědčí pro zapojení drah mozečku do sekvencování pohybů a podporuje koncept kognitivní dysmetrie.

Abstract:

Subtle neurological abnormalities are frequent findings in schizophrenia and they are traditionally referred to as neurological soft signs (NSS). The most common NSS in patients with schizophrenia are movement sequencing abnormalities. The purpose of this study was to determine neurobiological correlates of movement sequencing abnormalities in schizophrenia patients and to test the hypothesis of Cognitive Dysmetria, i.e. abnormal cerebellar connectivity in schizophrenia. 24 patients with schizophrenia and 23 healthy controls matched for age, gender, and handedness participated in this study. Patients with schizophrenia were subdivided based on the NSS-SQ score into those with deficits in sequencing of complex motor acts, the SQabn group (n=7), and those with normal performance, the SQnorm group (n=17). Pedunculus cerebellaris superior (SCP) and corticospinal tract (CST) were reconstructed using DTI tractography and DTI parameters were compared among healthy controls, patients SQabn and patients SQnorm. We have found impaired integrity of the right corticospinal tract in schizophrenia patients without movement sequencing abnormalities (SQnorm). Impaired integrity of the left pedunculus cerebellaris superior was found in patients with movement sequencing abnormalities (SQabn). Moreover there was a significant correlation between DTI parameters in pedunculus cerebellaris superior and severity of movement sequencing abnormalities. In conclusion, tractography of DT-MRI data showed that schizophrenia patients can be divided into subgroups (SQabn and SQnorm) based on abnormalities of specific white matter fiber tracts. Abnormal pedunculus cerebellaris superior in SQabn subgroup demonstrates the involvement of the cerebellar fiber tracts in movement sequencing and supports the hypothesis of Cognitive Dysmetria.
