Comprehensive nursing process for a patient with lung cancer after lobectomy

Abstract:

Tématem této bakalářské práce je komplexní ošetřovatelská péče u pacienta s karcinomem plic po lobektomii. Práce je složena ze dvou částí, z části teoretické a praktické. V teoretické části je stručně charakterizován karcinom plic- jeho rozdělení, příznaky, možné příčiny a rizikové faktory, vyšetřovací metody napomáhající k diagnostice a současné možnosti léčby. Dále popíšeme hrudní drenážní systémy, které patří do nedílné součásti při léčbě po provedeném operačním výkonu. Poté si vyjmenujeme možné komplikace u pacienta s hrudním drénem a nejčastější pooperační komplikace. V praktické části si představíme úlohu sestry v péči o takového pacienta a také její kompetence. Přiblížíme si pojem edukace a její nejdůležitější témata u pacienta s hrudním drénem. Dále si vyjmenujeme edukační témata v péči o dýchací cesty a seznámíme se s nejvhodnějšími polohami vedoucími k usnadnění dýchání. Poté je následná praktická část věnována ošetřovatelské péči o pacienta po lobektomii. Cílem práce je představit komplexní ošetřovatelskou péči u pacienta s karcinomem plic, seznámit se specifiky péče o pacienta s hrudním drénem a poukázat na nutnost předcházení možných komplikací vzniklých po operačním výkonu. Samotný ošetřovatelský proces uvedeme formou kazuistiky, ve které rozpracujeme ošetřovatelské problémy a diagnózy aktuální i potencionální, zhodnotíme průběh hospitalizace. Důležitou součástí je též vytvořený edukační leták, který poskytuje nemocnému rady, které by měly být nápomocny pacientovi po ukončení hospitalizace a následném propuštění do domácího ošetřování. Na závěr zmapujeme výsledky práce, zda byly splněny všechny stanovené cíle a úkoly práce.

Abstract:

The topic of this bachelor thesis is complex nursing care in a patient with lung cancer after lobectomy. The thesis consists of two parts, the theoretical part and the practical one. In the theoretical part is briefly characterized lung cancer - its distribution, symptoms, possible causes and risk factors, examination methods to help diagnose and current treatment options. Next, we describe the thoracic drainage systems that belong to an integral part of treatment after surgery. Then we list possible complications in the patient with chest drain and the most common postoperative complications. In the practical part we introduce the role of nurses in the care of such a patient and also their competence. We will describe the concept of education and its most important topics in a patient with chest drain. We will also list educational topics in respiratory care and get to know the most suitable positions to help breathing. Then the following practical part is devoted to nursing care of a patient after lobectomy. The aim of this work is to present complex nursing care for a patient with lung cancer, to acquaint with the specifics of patient care with thoracic drain and to point out the necessity to prevent possible complications arising after surgery. We will present the nursing process itself in the form of a case study, in which we develop nursing problems and diagnoses both current and potential, and evaluate the course of hospitalization. An important part is also created Educational leaflet, which provides the patient with advice, which should help the patient after the end of hospitalization and subsequent release to home care.Finally, we will map out the results of the work, whether all the goals and tasks of the work have been met.
