Utilization of purified toxic compounds for nefrotoxicity study In Vitro

Abstract:

Acetaminofen (paracetamol, APAP) je v naší i světové populaci jedním z nejužívanějších analgetik a antipyretik. V ČR je registrováno přibližně 250 volně prodejných léků obsahujících tuto účinnou látku. Díky tomu každoročně narůstá riziko jeho předávkování, které může vést až k akutnímu poškození ledvin. Cílem této práce proto bylo charakterizovat nefrotoxické působení APAP a jeho konjugátů in vitro a vyvrátit či potvrdit námi uvažovanou hypotézu o možné roli APAP konjugátů v acetaminofenové nefrotoxicitě. Dále jsme se zaměřili na možné využití detekce extracelulární koncentrace neutrofilního s gelatinasou asociovaného lipokalinu (Hall et al.) při hodnocení akutní in vitro nefrotoxicity. Všechny experimenty byly testovány na lidských imortalizovaných proximálních tubulárních buňkách (HK-2 linie). K testování APAP nefrotoxicity jsme de novo organickou syntézou připravili glutathionový (APAP-SG) a cysteinový (APAP-Cys) konjugát acetaminofenu, u kterých byla po jejich purifikaci ověřena jejich struktura a čistota (> 98 %). U testování NGAL produkce byly k indukci nefrotoxicity použity různorodé chemické sloučeniny (APAP, terc-butylovaný hydroperoxid, cisplatina, HgCl2, CdCl2 a gentamicin). Charakterizací APAP nefrotoxicity jsme zjistili, že samotný APAP způsobuje na dávce a délce inkubace závislé poškození buněk, které je asociováno s produkcí reaktivních kyslíkových sloučenin (ROS), nikoli ale s poklesem intracelulární koncentrace glutathionu (GSH). Významný toxický vliv jsme nalezli také u APAP-SG a APAP-Cys, přičemž u APAP-SG jsme pozorovali zvýšenou tvorbu ROS spojenou s deplecí GSH. Při hodnocení nefrotoxicity HK-2 buněk pomocí NGAL jsme překvapivě zjistili, že míra toxického poškození buněk negativně koreluje s poklesem extracelulární produkce NGAL. Námi nalezené výsledky potvrzují náš předpoklad o významné roli APAP konjugátů v acetaminofenové nefrotoxicitě a zcela vyvrací obecně přijímanou skutečnost o jejich netoxickém vlivu. U hodnocení nefrotoxicity pomocí extracelulární produkce NGAL jsme popsali zajímavý vztah mezi nefrotoxicitou nalezenou pomocí běžných cytotoxických testů a jeho koncentrací.

Abstract:

Acetaminophen (paracetamol, APAP) is one of the mostly used analgesics and antipyretics in the Western countries. In the Czech Republic, more than 250 over-the-counter drugs containing acetaminophen have been registered in 2015. Due increasing incidence of APAP overdose, the occurrence of acute kidney injury increases too. Thus, the aim of our study was to characterize the nephrotoxic effect of APAP and its conjugates in vitro and to test our hypothesis on possible role of APAP conjugates in acetaminophen toxicity. During estimation of nephrotoxicity in vitro, we aimed also to evaluate the use of Neutrophil gelatinase-associated lipocalin (NGAL) as a marker of damage of kidney cells in vitro. The experiments were carried out on human immortalized proximal tubular HK-2 cells. We used de novo organic synthesis to prepare acetaminophen conjugates with glutathione (APAP-SG) and cysteine (APAP-Cys). The final purity of these compounds was > 98 %. To characterize the production of NGAL in HK-2 cells, we induced the toxicity using acetaminophen, tert-butylhydroperoxide, cisplatin, mercury, cadmium and gentamicin. Acetaminophen treatment caused dose and time-dependent impairment of HK-2 cells. The cell damage was associated with enhanced reactive oxygen species production but no decrease of glutathione levels. In addition, we proved significant toxic effect after APAP-SG and APAP-Cys treatment. The incubation with APAP-SG caused increased production of ROS again linked with glutathione depletion. Interestingly, the characterization of NGAL production showed that extracellular NGAL concentration correlated negatively with cellular impairment. Our results confirm our assumption on possible role of APAP-conjugates in acetaminophen nephrotoxicity and exclude the generally accepted view on their nontoxicity. Our unique results on decreased NGAL production in HK-2 cells and its comparison with cytotoxicity tests showed an interesting relation with cell impairment.
