Determination of plasma uracil for prediction of fluorouracil therapy toxicity

Abstract:

Fluorouracil (FU) je v klinické praxi běžně používaný protinádorový lék, který však může v některých případech způsobovat závažnou až letální toxicitu. Fluorouracil je stejně jako endogenní uracil katabolizován enzymem dihydropyrimidin dehydrogenázou (DPD), a proto je měření předléčebné hladiny uracilu jednou z možností identifikace pacientů s rizikem vzniku závažných toxických účinků terapie. Byla vytvořena LC MS metoda stanovení uracilu pomocí metody náhradního analytu. Náhradním analytem byl izotopově značený 13C 15N2 uracil. Analyt byl extrahován pomocí metanolu, metyl-terc-butyléteru a vody, chromatograficky separován na koloně Hypercarb™ s gradientovou elucí a analyzován pomocí tandemového hmotnostního spektrometru EVOQ Qube vybaveného elektrosprejem. Uracil byl kvantifikován v režimu negativních iontů. Celková doba chromatografické analýzy byla 11,4 minut. V navrženém rozsahu 1–100 ng/ml nebyla kalibrační křivka hodnocena jako lineární z důvodů absence interního standardu. Z tohoto důvodu také nebylo možné metodu plně validovat ve všech validačních parametrech.

Abstract:

Fluorouracil (FU) is a commonly used anticancer drug in clinical practice, but in some cases, it can cause severe or even lethal toxicity. Fluorouracil, as well as endogenous uracil, is catabolized by the enzyme dihydropyrimidine dehydrogenase (DPD), and therefore, measurement of pretreatment uracil levels is one of the possibilities for identifying patients at risk of developing serious toxic effects based on therapy. An LC-MS method for the determination of uracil using a surrogate analyte method was developed. The surrogate analyte was isotopically labelled 13C 15N2 uracil. The analyte was extracted using methanol, methyl tert-butyl ether and water, chromatographically separated on a Hypercarb™ column with gradient elution and analyzed using an EVOQ Qube tandem mass spectrometer equipped with an electrospray. Uracil was quantified in negative ion mode. The total chromatographic analysis time was 11.4 minutes. In the proposed range of 1–100 ng/ml, the calibration curve was not evaluated as linear due to the absence of an internal standard. For this reason, it was also not possible to fully validate the method in all validation parameters.
