Physiotherapy of painful shoulder syndrome

Abstract:

Úvod: Tématem této bakalářské práce je fyzioterapie u syndromu bolestivého ramene. Cíl práce: Cílem je přispět křešení problematiky volbou optimálních fyzioterapeutických postupů, konceptů a metod u pacientky se syndromem bolestivého ramene. Metodika: V této bakalářské práci je zpracována kazuistika ženy (69 let) se syndromem bolestivého ramene. Nejprve byl proveden vstupní kineziologický rozbor (anamnéza, antropometrie, goniometrie, aspekce, palpace, vyšetření kloubní vůle a zkrácených struktur, svalový test, speciální testy na ramenní kloub), na základě jehož výsledků byl stanoven krátkodobý plán fyzioterapie. S pacientkou jsem spolupracovala 5 týdnů, během kterých proběhlo 10 terapií. Ve 3. týdnu terapie bylo provedeno kontrolní vyšetření zaměřené na svalovou sílu, rozsah pohybu a specifické testy na ramenní kloub. Na základě výsledků kontrolního vyšetření jsme se s pacientkou v druhé polovině terapie zaměřily dále na zvětšení rozsahu pohybu a zvýšení svalové síly. Během léčby bylo využito následujících fyzioterapeutických metod a konceptů: techniky měkkých tkání, postizometrická relaxace, postfacilitační strečink, mobilizace, Brügger koncept, proprioceptivní neuromuskulární facilitace a další. Výsledky: Během 5týdenní fyzioterapeutické intervence došlo u pacientky ke zvětšení rozsahu v ramenním kloubu. Na počátku terapie byla aktivní abdukce 90°, zevní rotace 50°, vnitřní rotace 50°. Po ukončení intervence bylo dosaženo plného rozsahu pohybu v ramenním kloubu. Dále došlo ke zlepšení svalové síly u flexe ze stupně 4 na stupeň 5, abdukce ze stupně 3+ na stupeň 5, zevní rotace ze stupně 2 na stupeň 4+, vnitřní rotace ze stupně 3 na stupeň 4+. Odezněla i bolest klidová a během pohybu. Na počátku terapie hodnotila pacientka bolest na stupnici VAS 7 v klidu a VAS 9 během pohybu. Závěr: Během zpracování hlavní části bakalářské práce jsem si osvojila využití mnoha fyzioterapeutických metod a konceptů v praxi. Spolupráce s pacientkou byla velice přínosná díky její aktivní spolupráci.

Abstract:

Introduction: The topic of this bachelor thesis is physiotherapy of painful shoulder syndrome. The objective: The aim of the thesis is to contribute to the solution of the problem by selecting appropriate physiotherapeutic techniques, methods and concepts in the patient with painful shoulder syndrome. Methodology: In this bachelor thesis a case report of a sixty-nine years old woman with the painful shoulder syndrome is presented. First, an initial kinesiological analysis was performed, then based on the results of a short-term physiotherapy plan was defined. I worked with patient for 5 weeks, during which 10 therapies took place. In the 3rd week of therapy, a follow-up examination was performed, focusing on muscle strenght, range of motion and some specific tests on the shoulder joint. Based on the results of the follow- up examination, the patient and I focused further on increasing range of motion and muscle strenght in the second part of the therapy. The following physiotherapy methods and concepts were used during the treatment: soft tissue techniques, post-izometric relaxation, post-facilitation stretch, mobilisation, Brügger concept, proprioceptive neuromuscular facilitation and others. Results: During the 5week physiotherapy intervention full range of motion of shoulder was achieved. At the beginning of therapy was 90° into active abduction, 50° into active external rotation and 50° into active internal rotation. Furthermore, muscle strenght was increased in flexion from grade 4 to grade 5, abduction from grade 3+ to grade 5, external rotation from grade 2 to grade 4+ and internal rotation from grade 3 to grade 4+. Also the pain was eliminated. At the beginning, the patient rated pain on a VAS 7 at. The rest anf VAS 9 during the movement. Conclusions: During the practical part of my bachelor thesis I have learned to use many physiotherapy methods and concepst in practice. The collaboration with the patient was very beneficial due to her active cooperation.
