Stochastic models for prognoses of therapeutic results in patiens with chronic myeloid leukemia

Abstract:

Tématem mé bakalářské práce jsou Stochastické modely predikující výsledky léčby chronické myeloidní leukemie. Chronická myeloidní leukemie (CML) představuje 15% všech leukemií v dospělosti. Tuto nemoc způsobuje mutací chromozomů 9 a 22 – reciproční translokací, kdy se jejich raménka vymění a vzniká tak fúzní gen BCR-ABL produkující protein, který spouští řadu procesů pro tvorbu nových buněk (bílých krvinek). Tento chromozom se nazývá podle místa jeho objevení: Philadelphský chromozom. Bílé krvinky se pak díky aktivovaným procesům začnou nekontrolovatelně množit. Existuje spousta léčebných prostředků, mezi něž patří např. hydroxyuera, busulfan, v nejhorším případě transplantace kostní dřeně, která je bohužel ne u každého možná a ne každý pacient ji přežije. Nejnovějším lékem je ovšem imatinib, který funguje na principu inhibitorů tyrozinkináz, tzn., že potlačuje působení nového, zmutovaného genu. V současné době je však nejpoužívanější a nejúspěšnější lék. Má ovšem velmi nepříznivé nežádoucí účinky, které, pokud se projeví, musí se okamžitě řešit přechodem na novou léčbu. Bohužel se na něj u některých pacientů objevují také určité rezistence nebo mutace, takže léčba imatinibem ztrácí smysl a pacient je nucen opět přejít na novou léčbu (dasatinib nebo nilotinib, což je druhá generace inhibitorů tyrozinkináz). Inhibitory celkově ale neumí nemoc úplně vyléčit, protože po jejich vysazení se znovu objevuje. Model predikující výsledky této léčby bude čerpat ze dvou jediných datových souborů, kde jsou zachyceni všichni pacient s CML včetně České i Slovenské republiky. Bude z nich vytvořen model vlivů faktorů ovlivňujících dosažení léčebné odpovědi a přežití pacientů CML v parametru event-free survival. Je založen také na faktorech, jako je optimální, suboptimální odpověď nebo molekulární, cytogenetická, hematologická odpověď.

Abstract:

Enter my work are stochastic models predicting the results of treatment of chronic myeloid leukemia. Chronic myeloid leukemia (CML) accounts for 15% of all leukemias in adults. This disease is caused by mutation of chromosomes 9 and 22 - reciprocal translocation, when the reversing arm and thus the fusion of BCR-ABL gene produces a protein that triggers a series of processes for production of new cells (white blood cells). This chromosome is known as the place of its discovery: Philadelphia chromosome. White blood cells are then activated by processes begin to multiply uncontrollably. There are many treatments, which include hydroxyuera, busulfan, in the worst case of bone marrow transplantation, which is unfortunately not possible for everyone and not every patient survive it. However, the newest drug imatinib, which operates on the principle of inhibitors of tyrosine kinases, ie. Inhibiting the action of a new, mutated gene. Today is probably the most used and most successful drug. However it has very negative side effects, which, if it occurs, must be dealt with immediately transition to the new treatment. Unfortunately there are some resistance or mutations on it, so it loses its sense of imatinib therapy and the patient is forced to once again go to a new therapy (dasatinib or nilotinib, which is the second generation of inhibitors of tyrosine kinases). Inhibitors generally but can not cure the disease completely, because after their discontinuation reappears. The model predicting the results of this treatment will benefit from the only two data sets, where they are captured all patients with CML, including the Czech and Slovak Republic. Of them will be a model of the effects of factors affecting the achievement of response and survival of CML patients in the parameter event-free survival. It is also based on factors such as optimal, suboptimal response or molecular, cytogenetic, hematologic response.
