The role of physiotherapy after breast surgery

Abstract:

Cíl práce: Cílem práce je přispět k řešení fyzioterapie u žen po operaci prsu volbou vhodných fyzioterapeutických postupů, konceptů a metod. Metodika: Metodika sběru dat proběhla na základě vyšetření pletence ramenního aspekcí (držení, konfigurace, trofika) a palpací (hybnost, tonus, svalová síla). Výsledky vstupního vyšetření sloužily k sestavení krátkodobého plánu fyzioterapie - polohování, respirační fyzioterapie, CG, ošetření jizvy, TMT, PIR, PNF, ACT, McKenzie a Brůggerův koncept. Kontrolní vyšetření proběhlo 4. týden, dále 3. a 6. měsíc po chirurgickém zákroku. Hodnotila jsem především funkční vyšetření pletence ramenního. Terapie byla vždy cílena na aktuální problematiku - ošetření měkkých tkání, zvětšení ROM a svalové síly v pletenci ramenním apod. Po dalších šesti měsících jsem provedla výstupní vyšetření, podle něhož jsem pacientce stanovila dlouhodobý plán fyzioterapie. Během roční fyzioterapeutické intervence proběhlo celkem 12 terapií. Pro metodu zjišťování empirických dat jsem následně využila dotazníkového šetření, které bylo uskutečněno v Mamma centru Hradčanská představující screeningové pracoviště specializované na diagnostiku onemocnění prsu. Výsledky: U pacientky došlo během roční fyzioterapeutické intervence k navrácení plného rozsahu pohybu (vpravo: FL 90° -> 180°, ABD 90° -> 180°, VR 60°-> 90°) a svalové síly v pletenci ramenním (vpravo: FL 3+ -> 5, ABD 3+ -> 5, VR 3+ -> 5). Podařilo se nám také upravit pohybový a dechový stereotyp. Z dotazníkového šetření vyplývá nedostačující edukace pacientek v rámci pooperační fyzioterapie, stejně tak absence následné rehabilitace. S omezením ROM v pletenci ramenním se po operaci přitom setkala jedna pětina respondentek. Nejčastějším typem omezení hybnosti byla flexe, kterou označilo 100 % žen. Pět respondentek z dvanácti současně označilo omezení hybnosti do abdukce. Závěr: Spolupráce s pacientkou pro mě byla velmi přínosná, stejně tak vyhodnocení dotazníkového šetření. Mohla jsem se zabývat fyzioterapií s aktuální tematikou a upozornit na klíčovou roli fyzioterapie v oblasti mammární chirurgie.

Abstract:

Aim: The goal of this thesis is to solve the problems of choosing the optimal physiotherapeutic procedures, concepts and methods for women after breast surgery. Methodology: The methodology was based on the examination of the shoulder girdle including aspection (posture, configuration, throphics) and palpation (momentum, tone, muscle strength). The initial examination was used to create a short-term therapy plan - positioning, respiratory physiotherapy, scar treatment, STT, PIR, PNF, ACT, McKenzie and the Brůgger's concept. The follow-up examinations were performed 4 weeks, 3 and 6 months after the surgery. I primarily evaluated the functional examination of the shoulder girdle. The therapy has always been focused on the current problem - treatment of soft tissues, increasing ROM and the muscle strength in the shoulder girdle. After another 6 months, I performered the final examination and established a long-term physiotherapy plan. There were 12 therapies during the whole cooperation. Another methodology was a questionnaire survey, which took place in the Mamma Centre Hradčanská. Results: During the one-year physiotherapy intervention the patient has returned to the full ROM (right: FL 90° -> 180°, ABD 90° -> 180°, VR 60°-> 90°) and the muscle strength in the shoulder girdle (right: FL 3+ -> 5, ABD 3+ -> 5, VR 3+ -> 5). We also managed to improve the movement and breathing stereotype. The results of the questionnaire survey show the missing knowledge of patients in a postoperative physiotherapy and the absence of the subsequent rehabilitation. One fifth of the respondents encountered some limitation of the range of motion in the shoulder girdle after the surgery. The most common type of movement limitation was flexion, which was marked by 100 % of women. At the same time five of the twelve respondents marked the problem with abduction. Conclusion: Cooperating with my patient and the evaluation of questionnaire survey was very useful to me. I was able to deal with a current problem and draw the attention to the role of physiotherapy after breast surgery.
