Risk analysis in case of lack of nursing staff

Abstract:

Diplomová práce na téma Analýza rizik při nedostatku ošetřovatelského personálu je zaměřená na kvalitu poskytované ošetřovatelské péče, která je ovlivněna nedostatkem ošetřovatelského personálu a na rizika, která vyplývají z nezapočaté nebo nedokončené ošetřovatelské péče. Cílem diplomové práce bylo zjistit na vybraném pracovišti resuscitační péče jaká péče je nezapočatá nebo nedokončená, podle jakých kritérií se sestry rozhodují, jakou ošetřovatelskou péči neprovedou a zda si uvědomují, jaká rizika sebou neposkytnutá péče přináší. Dalším stanoveným cílem diplomové práce je zjistit, jaká rizika plynoucí z neposkytnuté ošetřovatelské péče jsou nejzávažnější a navrhnout opatření k jejich eliminaci nebo snížení na minimum. Pro splnění stanovených cílů byly použity výzkumné metody polostrukturovaný rozhovor, vlastní pozorování a pro stanovení nejzávažnějšího rizika byla použita analytická metoda FMEA. Z polostrukturovaného rozhovoru, byly pro výzkum získány potřebné informace od oslovených respondentů vybraného resuscitačního oddělení. Z jejich odpovědí je patrné, že nejčastější intervencí, která není prováděna v souladu s platnými standardy je polohování. Pomocí vlastního pozorování sester při jejich činnostech bylo ověřeno, že tato informace je pravdivá. FMEA analýza, kterou jsme na vybraném oddělení prováděli s týmem vybraných sester, odhalila jako nejzávažnější riziko katétrovou sepsi, přenos infekce spojené se zdravotní péčí a vznik dekubitů. U vybraných rizik jsme provedli podrobnou analýzu a navrhli nápravná opatření, která budou v budoucnu na vybraném oddělení implementována do praxe. Informace získané při výzkumu pro účely diplomové práce poukazují na nejzávažnější rizika, která mohou vznikat v důsledku snížené kvality poskytované péče, která je důsledkem velkého pracovního vytížení ošetřovatelského personálu. Metoda FMEA je vhodnou metodou pro management na liniové úrovni pro eliminaci rizik při poskytování ošetřovatelské péče a navržená opatření mohou být v praxi využita na ošetřovacích jednotkách stejného typu to znamená na odděleních resuscitační péče.

Abstract:

The thesis on Risk Analysis in case of lack of nursing staff focuses on the quality of nursing care provided, which is affected by the lack of nursing staff and on the risks arising from unedued or incomplete nursing care. The aim of the thesis was to find out at the selected resuscitation care workplace what care is not started or unfinished, according to which criteria the nurses decide what nursing care they will not perform and whether they are aware of the risks of care not provided. Another stated objective of the thesis is to find out what risks arising from non-provided nursing care are most serious and to propose measures to eliminate them or reduce them to a minimum. Semi-structured interview research methods, self-observation were used to meet the objectives set, and the FMEA analytical method was used to determine the most serious risk. From a semi-structured interview, I obtained the necessary information for my research from the addressed respondents of the selected resuscitation department. Their responses show that the most common intervention that is not carried out in accordance with the applicable standards is positioning. By observing the nurses myself in their activities, I have verified that this information is true. The FMEA analysis we conducted in the selected department with a team of selected nurses revealed catheter sepsis, transmission of healthcare-related infection, and the development of dekubits as the most serious risk. For selected risks, we have carried out a detailed analysis and proposed corrective measures that will be implemented in practice in the future in the selected department. Information obtained from research for the purposes of the thesis points to the most serious risks that may arise from the reduced quality of care provided, which is the result of the heavy workload of nursing staff. The FMEA method is an appropriate method for line-level management to eliminate risks in the provision of nursing care and the proposed measures can be used in practice in treatment units of the same type, i.e. in resuscitation units.
