Mutation analysis of the LQTS candidate genes

Abstract:

Poruchy srdeční činnosti představují jednu z nejčastějších příčin úmrtí v západních zemích. Náhlá srdeční smrt (NSS) je tedy klinicky i společensky významným jevem. NSS vykazuje familiální výskyt. Musí tedy existovat geneticky podmíněné odchylky fyziologických procesů, které riziko NSS zvyšují. Pro mutační analýzu byly vybrány geny SCN5A, KCNJ2, KCNE2, KCND3, C-terminální doména a spektrin vázající doména Ankyrinu-B. Jde o kandidátní geny pro LQTS, který bývá často používán jako model arytmogeneze. Pro mutační analýzu byly využity metody PCR, SSCP a TGGE. Analyzovány byly 3 skupiny subjektů (LQTS pacienti, non LQTS pacienti s arytmiemi a zdravé kontroly). V průběhu práce byly nalezeny tři alelické varianty genu KCND3 (C2645T, G669C a G375A). Alela A polymorfního místa G375A byla nalezena pouze u pacientů suspektních pro LQTS, v kontrolní skupině se nevyskytovala. V analyzovaných úsecích genů ANK2, KCNJ2 a KCNE2 nebyla nalezena žádná alelická změna. V genu SCN5A bylo nalezeno pět synonymních změn v kódující oblasti (G87A, G3183A, G4218A, C4509T a C5457T). Kromě G4218A byly již tyto změny v literatuře popsány jako polymorfizmy. V kódující oblasti genu SCN5A byly nalezeny také čtyři nesynonymní změny G101A (R34H), G142A (E48K), C6016G (P2006A) a A1673G (H558R). První tři změny byly nalezeny pouze v heterozygotním uspořádání. Polymorfizmus C6016G a A1673G patří mezi již popsané polymorfizmy. G101A (R34H) a G142A (E48K) jsou nově nalezené změny. C6016G byl nalezen jen ve skupině pacientů s non LQTS arytmiemi a to pouze u 5 jedinců. Podle našich nálezů tento polymorfizmus souvisí s délkou QTc. Díky použití intronových primerů se nám podařilo zachytit též tři změny v nekódujících oblastech genu SCN5A, které těsně sousedí s oblastmi kódujícími. První změna se nachází v intronu 9 (IVS9-3 C>A) druhá v intronu 24 (IVS24+53 T>C) a to pouze u kontrolní skupiny. Tyto dvě změny byly již popsány. Změna třetí se nachází v intronu 25 (IVS25+65 G>A) a popsána doposud nebyla. Analýza polymorfizmu IVS9-3 C>A ukázala, že v důsledku sekvenční změny dochází ke změně preferovaného akceptorového místa sestřihu. Heterozygotní varianta IVS9-3 C>A se obvykle vyskytovala společně s heterozygotní variantou A1673G (H558R), nebo homozygotní variantou 558R. Kombinace heterozygota pro IVS9-3 C>A a zároveň homozygotní sestava majoritních alel 1673A (H558) se vyskytla pouze u 3 pacientů suspektních pro LQTS. Může se jednat o náznak protektivní funkce alely G polymorfizmu A1673G (H558R). Při statistické analýze výsledků v rámci skupiny non LQTS arytmiků se objevily zajímavé souvislosti mezi polymorfními variantami genu SCN5A a hemodynamickými charakteristikami těchto pacientů s vážnými arytmiemi. Je tedy zřejmé, že význam synonymních polymorfizmů není zanedbatelný jak se myslelo dříve, ale že tento druh změn může hrát významnou roli v dotváření genetického pozadí závažných chorob.

Abstract:

Heart diseases belong to most common causes of death in Western countries. For this reason, the sudden cardiac death (SCD) is an clinically and socially important phenomenon. Clearly manifested familial incidence of SCD suggests existence of genetically determined variations of physiological processes increasing the risk of SCD. It is well known that mutations in myocardial ion channel genes are the cause of increased SCD risk in relatively rare diseases – LQTS and Brugada syndrome. These diseases pose a model for study of arrhythmogenesis on molecular level. The aim of the thesis was mutation screening of myocardial ion channel genes SCN5A, KCNE2, KCNJ2, KCND3 and C-terminal and spectrin binding domains of ANK2 gene, employing the PCR, SSCP and TGGE methodics for mutation analysis. Three groups of subjects were screened in order to assess the relationship of polymorphisms in above mentioned genes to SCD risk: SCD survivors (high risk group of patients with history of myocardial infarction who have survived mallignant ventricular arrhythmia), LQTS suspected patients and control group (normal population) Three allelic variants of KCND3 gene (C2645T, G669C and G375A) have been found. The 375A allele has been found only in the LQTS patients but not in the control group. No polymorphisms were detected in the ANK2, KCNJ2 and KCNE2 genes. Five synonymous changes were found in the coding region of the SCN5A gene (G87A, G3183A, G4218A, C4509T and C5457T). Except for G4218A, all these changes have been previously described as polymorphisms. Furthermore, four non-synonymous changes were revealed in SCN5A: G101A (R34H), G142A (E48K), C6016G (P2006A) and A1673G (H558R). The first three changes were found in heterozygous state exclusively. While the C6016G and A1673G belong to previously known polymorphism, the G101A (R34H) and G142A (E48K) are novel. C6016G has been found only in five non-LQTS patients. C6016G is a rare polymorphism probably associated with QTc. Using intronic primers, three intronic variants have been detected as well. The IVS9-3 C>A (intron 9) and IVS24+53 T>C) have been previously known and were found only in control group. The IVS25+65 G>A (intron 25) is novel. Analysis has shown that the IVS9-3 C>A sequence change is associated with an acceptor splice site preference. The genetic variations IVS9-3 C>A and H558R in SCN5A appear to co-segregate. Heterozygous IVS9-3 C>A usually appeared together either with heterozygous A1673G (H558R) or homozygous 1673G (558R), while the latter combination has been detected only in three suspected LQTS patients, which suggests possible protective function of the G allele of A1673G polymorphism. Statistical analyses have suggested possible associations of genotypes and alleles of SCN5A with hemodynamic characteristics of patients with severe non-LQTS arrhythmias, such as ventricular hypetrophy, troponine I serum level and blood platelets level. It is obvious that significance of synonymous polymorhisms is not as trivial as was thought previously and that they can play an important part in formation of genetic background of severe diseases.
