Complex angiogenesis monitoring in multiple myeloma

Abstract:

Angiogeneze je novotvorba cév fyziologicky se vyskytující při embryogenezi. V ovariích a endometriu v průběhu menstruačního cyklu a při reparačních procesech v organismu. Dnes je již jednoznačně ověřeno, že nádor o velikosti 1mm3 se nemůže dále zvětšovat, pokud není aktivována angiogeneze. Ta se postupně stává důležitým léčebným cílem a její ovlivnění pravděpodobně bude jedním z pilířů tzv. targeted theraphy (cílené léčby). Mnohočetný myelom je prvním maligním hematologickým onemocněním, u kterého byla prokázána zvýšená angiogeneze v kostní dřeni(1). K účelnému zapojení antiangiogenní léčby do terapeutických algoritmů v onkologii je klíčové změřit míru angiogeneze a její ovlivnění léčbou, o což se snaží následující práce.

Abstract:

Multiple myeloma (MM) is characterized by proliferation of malignant plasma cells that accumulate in the bone marrow and often produce a monoclonal immunoglobulin. MM was the first hematological malignancy in which increased angiogenesis rate was detected. Measurement of the bone marrow MVD is a standard method used to evaluate angiogenesis. It was repeatedly reported that MVD closely correlates with prognosis and clinical stage of MM. Angiogenesis activators thought to play a role in MM pathogenesis include vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF), and hepatocyte growth factor (HGF). They have been shown to stimulate the proliferation of malignant plasma cells as well as non-malignant stromal cells. Plasma concentrations of these cytokines correlate with the Durie & Salmon stage, but the results of studies published to-date are not conclusive as to their relative importance in pathogenesis and prognosis of MM. There is little information on the role of angiogenesis inhibitors such as angiostatin, endostatin, thrombospondin-1 (TSP-1), or platelet factor 4 in MM. So far, only one report has been published that showed increased levels of endostatin in a small cohort of MM patients. It has also been reported that angiostatin gives a reduction in HGF levels, inhibits neovascularization, and slows tumor growth in an animal model of plasma cell malignancy.
