Physiotherapy as part of a comprehensive treatment for pelvic floor dysfunction

Abstract:

Souhrn Cíl: Cílem této bakalářské práce je přispět k řešení dané problematiky volbou vhodných fyzioterapeutických metod, postupů a konceptů u pacientky s dysfunkcí pánevního dna. Metodika: Vyhledala jsem vhodnou pacientku, která se potýkala s diagnózou N 94. Bolest a jiné stavy sdružené s ženskými pohlavními orgány a menstruačním cyklem. Následně jsem odebrala podrobnou anamnézu, především gynekologickou, a to pomocí řízeného rozhovoru, provedla jsem celkový kineziologický rozbor. Dále jsem vyšetřila pánev, SI klouby a hluboký stabilizační systém páteře pomocí speciálních testů, které poukázaly na hlavní patologie. Využila jsem i vizuální analogovou škálu pro hodnocení bolestivosti menstruace. Na základě výsledků jsem stanovila krátkodobý plán fyzioterapie, který trval 12 týdnů, terapie byla realizována 1x týdně po dobu 60 minut. K terapii jsem využila TMT, mobilizace, speciální koncepty jako ACT, DNS, dále cviky dle Mojžíšové a cviky na BOSU?. Po ukončení celkové terapie jsem provedla znovu kineziologický rozbor, spolu se všemi testy a dle jejich výsledků jsem stanovila dlouhodobý plán fyzioterapie. Výsledky: Celkovou terapií jsme docílily značného zmírnění bolestí během menstruace, ale i bolestí zad. Dle VAS udává pacientka zlepšení z hodnoty 5 na hodnotu 2. Protáhla jsem thorakolumbální a dorsální fascie bilaterálně. Došlo ke zlepšení postavení pánve, tím se prodloužila délka PDK o 0,5cm a posunula se pravá gluteální rýha o 0,5cm vzhůru. Lepším postavením pánve se v určité míře zmírnila anteverze pánve a prohloubená bederní lordóza. Umbilicus se posunul do středu. Snížil se tonus u m. rectus abdominis, m. piriformis, u bránice a mm. erectore trunci mírně zvýšený tonus stále přetrvává. Svalová síla přímých břišních svalů se zvýšila z hodnoty 2+ na 3+, síla šikmých břišních svalů z hodnoty 3- na 3. Všechny testy na SI klouby se staly negativní. Nejvíce pozitivních změn se objevilo u vyšetření HSSP, kdy pacientka mnohem lépe zvládá aktivaci hlubokých svalů. Změnou je i dechový stereotyp, kdy se pacientka naučila používat brániční dýchání. Závěr: Pomocí terapie jsem zvládla pozitivně ovlivnit hlavní patologie pacientky. Z tohoto důvodu terapii považuji za správnou a účinnou. Navíc u pacientky došlo ke zlepšení psychického stavu.

Abstract:

Summary Goal: The aim of this bachelor thesis is to contribute to solving the given problems by selecting suitable physiotherapeutic methods, procedures, and concepts for a patient with pelvic floor dysfunction. Methods: I sought a suitable patient who was diagnosed with N 94 Pain and other conditions associated with fiale genitalia and the menstrual cycle. Then, I took a detailed anamnesis, mainly gynecological, using a controlled interview. Subsequently, I performed an overall kinesiological analysis. I also examined the pelvis, SI joints, and deep stabilization system of the spine using special tests that highlighted the main pathology. I also used a visual analog scale to assessment the pain of menstruation. Based on the results, I have established a short-term physiotherapy plan, that lasted for 12 weeks. The therapy was performed once a week for 60 minutes. I used TMT, mobilization, special concepts such as ACT, DNS, Moses' exercises and barefoot exercises. After completing the overall therapy, I re-performed the kinesiological analysis, along with all the tests. Based on their results, I established a long-term physiotherapy plan. Results: We have achieved the considerable reduction of pain during menstruation and back pain through overall therapy. According to VAS, the patient stated improvement from 5 to 2. I stretched the thoracolumbar and dorsal fascia on both sides. The pelvis improved, thus extending the length of the PDK by 0.5 cm and the right gluteal groove moved 0.5 cm upwards. By improving the position of the pelvis, the pelvic anteversion and deepened lumbar lordosis have been mitigated to some extent. The Umbilicus moved to the center. The tone decreased in m.rectus abdominis, m.priformis, in diaphragma and mm.erectore trunci slightly increased tonus still persists. The muscular strength of the abdominal muscles increased from 2+ to 3+, the strength of oblique abdominal muscles from 3- to 3. All tests on SI joints are negative. The most positive changes occurred in the HSSP examination when the patient was able to better control the activation of deep muscles. Breath stereotype of the patient was also changed. The patient has learned to use diaphragmatic breathing. Conclusion: I managed to positively influence the patient's main patology thanks to the therapy. For this reason, I consider the therapy to be proper and effective. In addition, the patient's mental state has been improved.
