Abstract:

Cíl: Cílem diplomové práce bylo retrospektivně zhodnotit vývoj a výskyt pádů pacientů na gerontopsychiatrickém oddělení v nemocnici. Metodika: Do souboru byly zařazeny všechny vyplněné protokoly hlášení "Nežádoucí událost - pád" pacientů, kteří v letech 2015-2018 během hospitalizace na oddělení upadli a splňovali základní kritéria zařazení do souboru. Konečný soubor byl sestaven ze 704 záznamů o pádu. Získaná data byla statisticky zpracována pomocí statistického programu Stata verze 14. Stanovené hypotézy byly vyhodnoceny pomocí chí - kvadrát testu pro dva a více výběru. Pro jeden výběr byl použit chí - kvadrát test dobré shody. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Ve zkoumaném období bylo na gerontopsychiatrické oddělení přijato celkem 2 990 pacientů mužů a žen. Zjistili jsme, že během každého roku upadlo 25 % přijatých pacientů. Vyšší výskyt pádů byl u žen 53 % a nejčastěji byl nahlášen pád v kategorii nad 80 let 64 %. Poměr zranění po pádu měli vyšší muži 65 %. Ve sledovaném období se zranilo 78 % mužů a žen z celkového množství 704 pádů, z toho 99 % pacientů bylo v riziku. Mobilita pacienta před pádem neměla vliv na počet pádů. Pády pacientů byly nejfrekventovaněji zaznamenány během dne. Místo s nejvyšším výskytem byl pokoj pacienta 39 %, a chodba 35 % jako okolnost pádů byla uvedena nejvíce nestabilita při chůzi 32 %. Závěr: Z retrospektivní analýzy dat vyplynulo, že pád pacientů ve zdravotnických zařízeních se stává, vlivem všech okolností spojených s hospitalizací, jednou z nejčastějších nežádoucích událostí v nemocnici. Proto je nutno se tomuto tématu věnovat. Následky pádů pro pacienta znamenají často fyzickou, ale i psychickou újmu s dopadem na kvalitu života. Pro zdravotnická zařízení pády znamenají zvýšení nákladů na léčbu a rehabilitaci. Proto je nutné přijímat preventivní opatření podložené současnými vědeckými poznatky a nastavit je tak, abychom incidenci pádu eliminovali.

Abstract:

Aim: The aim of the thesis was to retrospectively evaluate development and occurrence of falls of patients at the gerontopsychiatric department in hospital. Methodology: All adverse events of patient's fall between 2015-2018, that fulfilled basic selection criteria, were collected into a dataset. The final dataset consisted of 704 records of falls. Collected data were statistically processed by the statistical software Stata (version 14). Stated hypotheses were evaluated using chi - squared test for two and more samples. For one sample, a chi - squared test of goodness of fit was used. Statistical tests were evaluated at 5 % significance level. Outcomes: There were 2 990 men and women registered at the gerontopsychiatric department in the observed period. It was found that over the course of every year 25 % of registered patients fell. There was a higher occurrence of falls for women 53 % and most falls occurred within the age category of 80 years and more 64 %. The ratio of injuries after the fall was higher for men 65 %. In the selected period, 78 % of men and women were injured after the total amount of 704 falls, out of which 99 % of patients were at risk. The mobility of patients before the fall did not have any effect on the number of falls. The falls of patients were most frequently recorded during the day. Most falls occurred in the patient's room 39 % and the hallway 35 %. As a circumstance of the fall, an instability during walking was recorded the most frequently 32 %. Conclusion: The retrospective analysis showed that the falls of patients in healthcare facilities are influenced by all aspects of hospitalizations, one of the most common adverse events in hospital. This is a reason why we must pay attention to this topic. The consequences of the falls are often physical and even psychical harm with an impact on a quality of life. For medical facilities, the falls mean an increase in costs of treatment and rehabilitation. This is a reason why there should be preventive measures based on scientific knowledge, so the incidence of falls could be eliminated.
