Long-term physiotherapy management of a patient with cervical spinal cord injury and need for intensive respiratory care: case report

Abstract:

Among the most devastating damages affecting an individual's quality of life we certainly find spinal injuries. These lesions leave the cognitive and intellectual functions intact, but lead to many other disabilities, for example, the alterations of motility and sensitivity, the problems affecting the ventilatory and cardio-circulatory function, the alterations of the metabolism and gastrointestinal and bladder dysfunctions and genito-sexual. The patient with spinal cord injury must undergo a type of comprehensive rehabilitation that involves surgery dysfunctions. The rehabilitation of the patient with spinal cord injury is global, i.e. it requires multidisciplinary interventions, with the aim of regaining all the vital functions of the individual, which must be carried out in the context of team work. I participated in Sara's rehabilitation program (20 years) who reported spinal cord trauma having a dislocation of the intervertebral disc between C3 and C4, with fracture of the right peduncle of the underlying vertebra, medullary edema and canal stenosis. The fundamentals of anatomy of the cervical spine are initially described with a classification of the vertebrae, spinal cord and ligament structures present in the spine of the spinal column. I subsequently illustrated the main pathologies affecting the spine, grouping them into two classifications: traumatic and non-traumatic injuries. I have presented epidemiological studies with the relative incidence and prevalence data in Italy and worldwide. Finally, I will describe the clinical case during which neuro-muscular exercises are performed in a complementary way to the application of important innovative robotics aids which are the exoskeleton, the end-effector and the hunova which have determined a progressive improvement along the way rehabilitation. The application of this method has made it possible to act in specific muscle areas such as the abdominal wall for the stabilization of the trunk and the small, medium and large buttocks for achieving earlier verticalization.

Tra i danni pi? devastanti che incidono sulla qualit? di vita di un individuo ritroviamo sicuramente le lesioni al midollo. Tali lesioni lasciano integre le funzioni cognitive ed intellettive, ma conducono a molte altre disabilit? si pensi ad esempio alle alterazioni della motilit? e della sensibilit?, alle problematiche a carico della funzione ventilatoria e cardio-circolatoria, alle alterazioni del metabolismo e alle disfunzioni gastrointestinali, vescicali e genito-sessuali. Il paziente con lesione midollare deve essere sottoposto ad un tipo di riabilitazione globale che preveda interventi multidisciplinari, con l'obiettivo riacquisire tutte le funzioni vitali dell'individuo, che devono essere svolte nell'ambito del lavoro in equipe. Ho partecipato al programma riabilitativo di Sara (20 anni) che ha riportato un trauma midollare avente una lussazione del disco intervertebrale tra C3 e C4, con frattura del peduncolo destro della vertebra sottostante, edema midollare e stenosi del canale. Vengono descritti inizialmente i fondamentali di anatomia del rachide cervicale con una classificazione delle vertebre, del midollo spinale e delle strutture legamentose presente nel rachide della collona spinale. Ho illustrato successivamente le principali patologie a carico del rachide, raggruppandole in due classificazioni: le lesioni traumatiche e non traumatiche. Ho presentato studi di tipo epidemiologico con i relativi dati di incidenza e prevalenza in Italia e nel mondo. Infine descriver? il caso clinico durante il quale vengono svolti esercizi di natura neuro-muscolare in maniera complementare all'applicazione di importanti ausili di robotica innovativi che sono l'esoscheletro, l'end-effector e hunova che hanno determinato un progressivo miglioramento durante il percorso di riabilitazione. L'applicazione di questa metodologia ha permesso di agire in specifici distretti muscolari quali la parete addominale per la stabilizzazione del tronco e il piccolo, medio e grande gluteo per il raggiungimento pi? precoce della verticalizzazione.
