Assessing the potential health effects of human exposure to chemicals and other exposome factors

Abstract:

Lidé jsou denně vystaveni nesčetnému množství faktorů životního prostředí. Přesné hodnocení těchto expozic je proto stěžejní pro zodpovědné hodnocení zdravotních rizik a možných účinků na zdraví člověka. Existuje několik metod hodnocení expozice s různou přesností a komplexností. V nedávné době získal na popularitě lidský biomonitoring, což je metoda hodnocení vnitřní expozice využívající kvantifikace chemických látek, jejich metabolitů nebo jiných biomarkerů v biologických matricích. Tato metoda může být navíc doprovázena analýzou biomarkerů účinku a/nebo biomarkerů citlivosti. Zastřešujícím cílem této disertační práce je tedy provedení hodnocení vnitřní expozice chemickým látkám přednostně v české populaci, použití těchto dat pro hodnocení zdravotních rizik, a propojení s dalšími relevantními zdravotními údaji nebo informacemi z dotazníků. Za tímto účelem je PhD výzkum konstituován kolem pěti studií publikovaných ve vědeckých časopisech, které společně tvoří podstatu této disertační práce. Celkově, výsledky potvrzují expozici současně používaným pesticidům v české populaci s určitými věkovými a sezónními rozdíly. Využití dat o interní expozici pro hodnocení zdravotních rizik za použití reverzní dosimetrie doprovázené údaji o biomarkerech účinku naznačuje možná zdravotní rizika v hodnocené populaci, souvislost s oxidativním stresem a biomarkery epigenetických změn. Alternativní přístup využití dat lidského biomonitoringu v oblasti hodnocení zdravotních rizik je předveden na studii per- a polyfluoroalkylových látek v mateřském mléce. V rámci tohoto přístupu jsou informace o interní expozici jedné populace (kojící matky) použity pro externí hodnocení expozice v jiné populaci (kojené děti). Výsledky poukazují na potenciální zdravotní riziko pro kojence. Další analýzy vztahů s relevantními daty shromážděnými pomocí dotazníků, lékařských záznamů nebo místními charakteristikami ověřily jejich schopnost posloužit jako základ pro identifikaci expozičních cest, zdrojů expozice, potenciálních nepříznivých zdravotních účinků v rané fázi apod. Tato disertační práce zdůrazňuje důležitost studií lidského biomonitoringu ve vztahu k možným zdravotním účinkům. Výsledky práce naznačují naléhavou potřebu stanovit vhodné, zdravotně podložené, orientační hodnoty tak, aby bylo možné zodpovědně hodnotit přidružená zdravotní rizika. Tato výzva je ještě důležitější jsou-li brány v potaz zranitelné části populace jako například děti nebo kojenci. Využití biomarkerů účinku a pozorované asociace v této disertační práci umožňují další rozsáhlý výzkum znečišťujících látek životního prostředí a těchto biomarkerů.

Abstract:

Humans are exposed to myriad environmental factors on daily basis. Therefore, accurate assessment of such exposures is crucial to responsibly evaluate possible health effects and associated health risks. Several exposure assessment methods with different precision and different complexity exist. Recently, human biomonitoring, internal exposure assessment method employing quantification of chemicals, their metabolites, or other biomarkers in biological matrices, has gained its popularity. In addition, this method can be accompanied by analyses of biomarkers of effect and/or biomarkers of susceptibility. The overarching aim of this PhD thesis is to perform internal exposure assessment to chemicals preferably in the Czech population, to utilize such data in health risk assessment, and to connect them with other relevant health and/or questionnaire data. For this purpose, the PhD research is constituted around five studies published in scientific journals and forming the essence of the PhD thesis. Overall, the results confirm exposure to current-use pesticides in the Czech population with certain age and seasonal differences. Utilization of internal exposure data in health risk assessment using reverse dosimetry accompanied with effect biomarkers data suggests possible health risks in assessed population and associations with oxidative stress and epigenetic changes biomarkers. An alternative approach of human biomonitoring data integration in health risk assessment field is demonstrated via study of per- and polyfluoroalkyl substances in human milk. In this approach, the information about internal exposure of one population (breastfeeding mothers) are used for external exposure assessment in another population (breastfed infants). The results point to a potential health risk for breastfed infants. Further analyses of associations with relevant data gathered from questionnaires, medical records, or location characteristics prove its ability to serve as a basis for identification of exposure pathways, sources, potential adverse health effects at an early stage etc. This PhD thesis highlights the importance of human biomonitoring studies in relation to possible health effects. The results indicate an urgent need to establish appropriate health-based guidance values in order to responsibly evaluate associated health risks. This challenge is even more important when considering vulnerable population subgroups, such as children or breastfed infants. Utilization of effect biomarkers and observed associations warrant further large-scale research of these biomarkers and environmental pollutants.
